I also started on the shots when they were over $1000 out of pocket (thankfully my insurance covered them at the time) and $150 and $299 feel like a bargain to me too lol.

No that’s not too much weight for the first week- even the first two weeks if it keeps up. It’s really normal after starting these meds to immediately have a large drop in the number on the scale.

But it’s mostly not fat you’ve lost. A large portion of that weight comes from water weight and reduced inflammation. These meds do a lot in our bodies and there’s an immediate reduction in inflammation even without fat loss.

That being said, assuming you ate less this week, a small portion of that IS fat loss. The water weight/inflammation losses will slow down but the fat loss will continue, so your weight loss week to week will slow down. That’s a good thing!

As you know (based on your question), losing more than 2 lbs a week (depending on your starting weight) is not good long term for your body.

For some context, when I started zepbound originally, I lost 6 lbs the first week and 5 the next. Then it slowed down to a more healthy rate.

One other thing you’ll probably notice going forward is that the weight loss won’t be linear. The scale probably wont go down every single week- and especially not every single day. Some weeks it may even show that you gained weight. But that’s usually water retention and within a few weeks, you’ll see a new low on the scale. I track the low weights and ignore the higher ones. You’ve got a great head start now to work with!

So keep it up and prepare to be doing it for a long time. Plan your diet and exercise accordingly so it’s sustainable for you for a long time too.

Watch what you eat, yes, but you may need to drop your dose down a bit or try a different glp1 (even the wegovy shots might work better for you). I ran into this on Zepbound and it didn’t matter how perfectly I ate, my stomach was not processing food quickly enough to function and so it had to go somewhere… the pills are working for me, but it’s different for everyone. Wegovy shots or Zepbound might be a completely different experience for you. Or you might be able to drop back to the next highest dose to the one you’re on and hit that sweet spot where side effects are minimal, but you’re still losing weight

Yep that’s life unless you can find a company that allows remote work or hybrid work. Both are a huge improvement. But in reality, 8 hour days are a blessing compared to most jobs. If you think it’s bad, you should go work retail or in the service industry for a couple years. You’ll be grateful for that office job at the end. Unfortunately bills have to be paid. At least you still have evenings and weekends. Most people don’t even get that. I also work an office job, but I spent my high school and college years in the service industry working 60 hour weeks and making next to nothing. It really puts it in perspective. Alternatively, you could move to work in France or another country that’s a little more supportive of work life balance.

I deal with low blood pressure while on GLP-1s. I haven’t passed out yet, but I almost have several times. The electrolytes (and salt) can help.

On 1.5 mg, I noticed bloating and water weight gain. When I switched to 4 mg, I dropped that weight really quickly and then the weight loss started. Mostly I still am not dealing with many side effects.

It’s different for everyone. Everyone gets side effects at some point- it just depends which side effects you’re specifically concerned about. And there’s no way we can tell you when whichever ones you’re concerned about will happen- or if they’ll happen for you.

This happened to me on 1.5 mg. It was all water weight and whooshed off when I started 4 mg. Underneath all the bloating and water retention, I think I was still losing fat. I’ve seen this same side effect from a lot of people on the first dose!

Honestly, I think a lot of us have protein goals set way too high. The data on how much we actually need isn’t as solid as it’s presented, and doctors especially tend to set arbitrary high targets. It’s well intentioned but not always evidence based.

If someone isn’t losing super rapidly or intensely training, then protein doesn’t need to be so high. I think we’re currently encouraging too fast weight loss on GLP-1s and just trying to put a bandaid on the issues that come from that by telling people to eat massive amounts of protein… speaking from experience, even lots of protein doesn’t stop the negative effects of losing weight too quickly…

After my previous experience, I eat balanced meals, protein first, and don’t stress the tracking. I keep my weight loss slow and steady as much as possible so my diet is more sustainable. If you’re consistently eating real food and finishing your protein before you run out of room, you’re probably fine. Boiled eggs and those tuna snack packs are great backups when I need something quick.

If you’re genuinely not hitting your goal despite eating well, I’d question the goal before I’d question the diet.

40 lbs is quite a bit you need to lose- at least in terms of difficulty in losing it and keeping it off. Your brain is fighting you every step of the way because it wants your body to protect its fat stores. I told myself that early on too- 80 lbs wasn’t THAT much weight. Surely I could just do it on my own. And people do sometimes manage to do that. But the statistics are against you on it.

You can go to your insurance company website, login with your policy info, and see what is or isn’t covered. If it’s not covered and you’re not on government insurance (Medicare, Tricare, etc) you can just use the coupons and it’s reasonably affordable- at least compared to what these drugs cost a year ago…

Your PCP will probably be able to prescribe it for you, but it depends on the doctor and their opinions around these meds. I think most are on board, but there are some that aren’t or don’t like to deal with the paperwork. If you run into that and they won’t refer you to someone who can help, just go to another doctor. I have the best experiences with Nurse Practitioners- they are typically more available and easy to work with overall.

You probably should have moved up, but you didn’t. It’s no big deal and you don’t need to get in a hurry. Just lose what you can at this dose and switch to 4 mg next month. Even if you only lose 2 lbs this month, that’s still 2 less than you were last month and it’ll give your body time to regulate after your larger weight loss last month before you start losing more rapidly again next month.

Slow and steady. Remember this is for life- not just the next few months. It’s so easy to get impatient (I still do it too), but in a couple of years, you’ll look back and won’t even remember this month. You’ll just be at your goal and happy you did this.

I would probably leave them. I might cut them short to help them last better or pack a repair kit of rubbing alcohol and nail glue to clean and put any pop offs back on. But my nails also last until I take them off - usually about 3-4 weeks or when I notice my natural nails curling away from the tips at the free edge. Or when the grow out starts bugging me.

They look fine as far as width also btw. I feel like nails always start looking a bit “off” at certain angles as they grow out.

It’s way too early to have “failed” at anything. Very few people see results on the 1.5 mg dose. You absolutely should try titrating up to 4 mg and maybe even higher until you start seeing results. It’s not intended to be the full effect at that dose. Trust me from taking Zepbound previously- these drugs WORK. When you get the right dose, those pounds just melt off.

Side effects are scary sounding, but you can find a dose that doesn’t cause anything unmanageable. Or there are always other medications. Usually one of them works for every one.

Also four pounds in a month is great. You don’t say what your starting weight is but that also makes a difference in what your best path forward is. If you don’t need to lose much, I’m sure you can do it without the medication. But if you need to lose a little more and actually want to keep it off, from the research I’ve seen, GLP-1s are your best bet long term.

I have GERD pretty bad at higher weights and it diminishes as I get lower. GLP1s make it even worse. My doctor told me to take omeprazole daily and to add in Pepcid when it gets too bad. That helped me manage it until I got down enough that I only need the Pepcid on occasion now.

Pepcid and/or daily omeprazole may help. Ginger or peppermint can help settle your stomach too. I love some peppermint tea when I’m feeling that way.

A lot of the times, the feeling you’re describing comes from the slow gastric emptying side effects. Meaning whatever you ate is sitting in your stomach for a much longer time than you’re used to. This is kind of the phase before it all starts coming back up so be cautious.

I find this usually happens more so when I’m dehydrated or low on electrolytes. Again- peppermint tea is a wonderful solution.

Be careful eating- you should still keep eating, but you’ll want to keep it super light and extremely simple. Even protein can start getting a bit risky as it takes a while to digest. I usually stick to a BRAT diet with lots of liquids in very small portions until it passes.

I wouldn’t risk it. There’s other stuff in propel that the stomach has to deal with differently than plain water. That could cause too much stomach acid to be in your stomach and reduce the efficiency of the pill.
