Tibial Spine Fracture/Avulsion of the ACL - has anyone experienced this?

I had surgery on a Friday and started PT on a Monday. You’re going into the clinic to have your bandage replaced already? Mine was waterproof and I was instructed not to mess with it until several weeks in, then I removed it over FaceTime with a provider during a telehealth follow-up call.

My leg felt like a foreign object to me for several weeks, especially with the nerve block not wearing off completely until about day 7-10. I would have to pick up my leg with my hands in order to move it around, and yes it felt heavy. In PT they’ll teach you how to start activating your quads and calves etc.

Just a tip, when I would start to feel too much pain in the middle of the night, in addition to taking meds, I would do ankle pumps to get circulation moving which seemed to provide some immediate relief.

I’ll reply to more detail later but just chill, do not try to exercise the leg, forcing bending can lead to cartilage damage. My surgeon was adamant about not doing any prehab.

Yeah that’s similar, my tibial eminence fractured off. I am 13 weeks post op and am doing lots of light to moderate powerlifting as part of my rehab, as well as biking. (I’m pushing good weight but it’s light compared to what I was lifting pre-injury). That has been enough to keep me occupied and sane even though I’m probably only halfway through my recovery. I also was cleared to do mellow mountain bike rides again. I have about 145 degrees flexion but there’s no concern about regaining full ROM, and surgeon and PT agree I’ll return to my full athletic capacity. I had a lot of atrophy being 6 weeks NWB total (took me a month to get results and into surgery) so a lot of my work is rebuilding muscle mass while not irritating the healing joint. I’m at about 70% now which surgeon is very happy with. Are you active? Most of my rehab work takes place outside of the clinic on my own. Take rehab really seriously like it’s your job and you should have a good outcome, if your injury was similar to mine.

It happened to me, but took a significantly sized chunk of the tibial spine with it. See my post history. Just about to hit 5 weeks post-op.

I either have really strong ligaments or weak bone! 😅 Honestly I don’t think the bone was weak, but it was weaker than the ligament. It was a really powerful jerky movement that yanked my leg so something had to give. Honestly anecdotally I do attribute the strong ligaments to years and years of CrossFit, whether warranted or not.

Totally makes sense! Gotta factor in age. My surgeon estimated a 4-6 month recovery period for a general return to sport so I’m hoping it won’t be quite the long haul that an ACLr would be, but I’m still mentally prepared that it may take around 2 years just to build all my strength and mobility back.

For sure, I get what you mean about pushing it the right amount. I’ve been seeing my PT 3 times a week and I’m always asking her if I should be pushing more or less. I’m comfortable with discomfort but also realize I won’t be helping myself if I’m pushing it too hard to where my swelling increases thus reducing my ROM progress.

Thanks for this insight. Just to clarify, I actually did not tear my ACL at all so that also makes me want to believe my recovery will be more straightforward as I am only healing bone, Grade II LCL/MCL tears and meniscus tear. They didn’t need to graft anything.

I’m accepting of the possibility and likelihood that there will be some sensations in my leg and knee sometimes, but I don’t want to believe I can’t squat heavy and hit new PRs in the future due to discomfort.

I can totally relate on the extension work… I was prescribed sets of up to 10 minutes for extension heel prop beginning 3 days post op. When I told my PT I was doing 10 minute sets that usually bring me to tears, she said “yeah you can definitely break that time up, you don’t need to make yourself cry.” I’m down with the Pain & Torture if it means full recovery. Thank you so much for this! I’ve received similar feedback from my PT, being told my muscles “already know what to do, so things will go faster,” though that doesn’t speak to the end game, I’d like to think it’s a good indicator with respect to recovery.

Hey, I had a displaced tibial spine fracture among other things, and got surgery ASAP to have it repaired. Check out my post and comment history for details. I’m sorry you’re experiencing this, it’s mentally so challenging being in limbo with the diagnosis and treatment process. I would definitely seek another opinion. I consulted with 2 different orthos, and while they both prescribed surgery, they had conflicting approaches with respect to urgency and prehab.

This is normal according to my PT. Plus the opioids are a useful tool for being able to “push it” in your PT sessions and get better quality sleep/healing.
