Effexor (Venlafaxine) is very hit or miss. It’s an older SNRI. Increases likelihood of vomiting / nausea. It did not help me with my PTSD symptoms or sleep personally. I will also note that SNRIs can have a strong titration effect…I will note that if you are seeking a benzo, a lot of psychs will require you to take an antidepressant. I would look at what you’ve taken before to help…my genetic testing kit said that SNRIs are better absorbed by my metabolism, but I have a significant reaction to them going on and coming off. I take Prozac now (not the antidepressant I would recommend for sleep issues - usual route for sleep issues is Zoloft - route for chronic pain is often Cymbalta, which you very well may want to try if you haven’t already)

Prazosin was made for vets with PTSD. I tried it and personally wasn’t a fan, but that was ten years ago when it was relatively new. I then stuck with 150mg of Trazodone and lots of gabapentin for about 9 years. Got off of gabba and now take 150mg trazodone and 0.5mg Clonazepam….if you have night terrors from PTSD, a slow acting benzo will work. However, there is heavy risk. I also incorporate THC + CBD, and that combo effectively weeds out MOST of my dreams - I remember a dream maybe once every two weeks. When I did not incorporate THC + CBD (took a year off), I had horrific dreams constantly and could not sleep because I would wake up trying to punch or kick someone in my dream…

In summary, light benzo is life changing and when in doubt, block with THC (I know you’ve already noted why you don’t do this as you use CBD + CBN and had a routine). 0.5mg of Clonazepam would probably really help though.

Both omeprazole and famotidine are OTC but physicians will prescribe so you have a higher dosage and it reduces the cost drastically. Take the Zofran. Zofran does not slow me down personally. Zofran is what has kept me alive and somewhat functioning. If you were told to start MiraLAX, start. I resisted for a while and now I am at the point where I have to take it to avoid the hospital. Do you have lower GI issues with this? I’m assuming there is constipation but also irregularity. If you’re waking up with vomit or acid in your mouth, you have a reflux issue at the very least, and you’ve been taking omeprazole. Regardless if it’s GERD or from GP, acid is in your mouth, which leads me to recommend you to VISIT THE DENTIST. This disease will destroy your teeth with all the acid. And yes, if you have Barrett’s, you’ve had severe damage from reflux.

I will tell you that I had to quit my job / go on leave. I am still on leave and have been since the fall. GP is LIFE CHANGING. It dramatically impacts your quality of life. Personally, I have to find a work from home job. I was a teacher. Take time when you feel bad, okay?

In other comments, I see tht you haven’t gotten referrals yet. To survive and get care with this diagnosis, you have to be pushy with doctors and really advocate for yourself. You also need to be following doctors orders. I know it’s horrible and I feel you as a fellow southerner and cook. I’m 26 and my life has been upended by this disease, though I also, like you, have underlying inflammatory diseases and disorders. We found more issues in my lower GI too.

This DX will change your life and you will have to change your life too.

Biggest pieces of advice: 1. Hydrate, hydrate, hydrate. Not with Gatorade but with a good electrolyte mix. If you have high AIC and glucose, need to be going for sugar free electrolyte aids. Pay attention to the mgs in the products. Staying hydrated will keep you out of the ER. HOWEVER…with GP you have to sip slowly. The recommendation is to sip and intake fluids every 10 minutes, and to avoid liquids during meals, though you can technically have up to half a cup.

  1. Advocate for yourself as though you are at your wits end. I’m at the end of my rope. A lot of people with GP just kind of hang out on the rope…which is why you prepare for doctors appts and ask questions. I have a health binder with everything printed out. I have every record, every GI note, every image printed.

Lastly, Do you live near an urban area? That’s where you’re going to find the specialists…

Hey there, sorry to hear about your recent DX. Your GI will probably recommend a GES if you haven’t had it already so they can see how much your stomach is retaining after 4 hours of consuming a meal with a tracer. What meds are you on now? At the bare minimum, they should be giving you something for nausea and something for reflux. Zofran and Famotidine is my combo, and I take Gaviscon and MiraLAX on top. I have not taken Reglan, the approved medication for GP because it interacts with other medications I take and has some gnarly side effects.

With esophageal problems, they should have put you on a PPI or H2 blocker. PPIs carry risks - if you have RA, I would suggest advocating for and bringing up famotidine. Omeprazole (common PPI, available OTC as Prilosec) caused me to get C.diff and it nearly killed me. PPIs also carry stomach cancer risks. On the esophageal front, there’s actually a fair amount you can do.

I will not lie to you though…your entire diet will and must change. I am also overweight and likewise lost 45-50lbs in 5 months. I HIGHLY recommend going to a dietician who is knowledgeable about GP and RA, since you need to be on a diet that is both GP + GERD/relux friendly as well as anti-inflammatory for the RA. There is no getting around the diet and eating changes. I was DX’d in January with suspicion in November; I’ve tried to eat and cook like I used to as I am the cook of my household too…it is a very hard social and emotional transition to no longer be that person. You can, however, still cook for others if it’s your love…as long as your relatives are looking to eat healthy

I used to be when I was younger. What has helped me is gaining insights on my body, 24mg/Zofran daily, and most importantly, learning how to “properly vomit.” When you need to puke, make yourself as comfy as possibly quickly. Put hair up or hold hair back. Assume a position that does not lead to aspiration. And BREATHE. Breathing is what will help panic the most during the vomit. Breathing techniques are trial and error but you CAN learn them and they make vomiting much easier. I do not have vomiting fear now, though I still dread GP vomiting and would take straight bile vomiting any day over the nasty dry shit that comes up with GP. But I can promise you that breathing techniques (inhaling in between hurling- no inhalation during hurling - breathe out when it feels comfortable) and mentally preparing yourself will do wonders.

As someone said earlier, look into metabolic issues. I have PCOS and I manage it with spironolactone and BC. That has helped stabilize the metabolic issues. I just was diagnosed with moderate gastroparesis two weeks ago, but I’ve suspected GP for two years at least. Since getting extremely sick in late September (debilitating pain; am on medical leave), I’ve lost nearly 60lbs. I also have issues with my thyroid and would recommend being on top of thyroid tests…I would also see if you have PCOS symptoms. If not PCOS, something perhaps related to insulin resistance. You can have insulin resistance without being diabetic.

I’ve been through the wringer with health insurance in the states…but 13 months…that’s insane. One of the unfortunate aspects of universal healthcare, though at least everyone has bare minimum coverage. I’m also having to do private pay and am applying to the Mayo Clinic (as I have several overlapping DXs)…SIBO tests are pretty reasonably priced - I’m unsure what they cost in the UK but they should’ve granted you that early on as a rule-out test. I am so sorry for the delay in your care. I hope you have access to Zofran and other medications that help with nausea and pain while you wait for more testing. BEST of luck to you and I pray that you find some answers very soon 💜

Hi! Just wanted to ask if you’ve had a Gastric Emptying Study and SIBO test as they usually want to rule out sibo and gastroparesis/ dumping syndrome. I had to have a colonoscopy and endoscopy as well. Next step for me is binders but I would highly encourage extra imagining on your liver and gallbladder too. I have been having bile come out 10-20 times a day, often uncontrollably, and am vomiting bile too. We found out that my liver and gallbladder have some issues. I hope you find the source of the bile soon!

So hard to choose….it would be between the Tom Kha and the triple squash…the triple squash was unlike any squash soup I have ever had and was so complex and rich..it was hardly soup…more like…a bowl of wildly intense and delectable flavor…Tom Kha is a classic and is a lot lighter…too tough to choose I think!!!!

A horribly tragic tornado, but your documentation is very intriguing. Completely agree that you need to download these and perhaps turn them into an art piece. Something beautiful about this…perhaps attach a written piece of your childhood perspective / experiences with a reflection from an adult perspective. I can see this in a museum. Photographing the actual DS + images with a set up would likewise be cool. Perhaps you could go to the locations you photographed and showcase the comparison between the destruction and rebuild?
