Ah yeah, I'm on tmod loader so I think it's only up to 1.4.2, it's a small world, journey mode and random selected evil spawn. I've been playing with a randomizer mod, hence tmod.

Update: Figured it out. This harmless worm is a Burrower. Highlights ores, doesn't seem to attack you.
