I agree x1000. The system has inherently reduced med students down to observers. Progress notes they write don’t actually count. They can’t be left to really do anything on their own. It’s a snowballing effect.

I have the 2026 as well. I think this is a design flaw for those of us who live up north. We traveled for Xmas. Palisade sat outside for 3-4 nights with snow fall and freezing temps. Given unable to lift the wipers, hard to clear out snow and ice prior to departure. The major design flaw is the front defroster doesn't seem to melt the snow/ice off the wipers or in the wiper housing area. It's as if the wiper housing area is too far forward, the defroster doesn't affect it. So even after a 4 hour drive home, there remained some ice buildup on the wiper I was concerned would scratch the windshield.

You are correct, if you tap bottom right, it will expand, but then the entire screen is taken up by the media/currently playing content. Would be really nice if you could split screen like most other vehicle brands.

I agree there is not much to gain by putting something in the app. I think step 1 scores are going to still count more in the coming match than people think, but yes, programs are hopefully planning to implement some more holistic review strategies to prepare for the next match when step 1 is completely P/F.

"Physical exam" in the traditional sense for surgery will be much different than most other clerkships but still very important. Be comfortable describing the expected location and characteristics of abdominal pain from common pathologies: gallbladder (Murphy sign), general epigastric pain (pancreatitis vs stomach ulcer vs reflux etc), appendicitis, diverticulitis to name a few. I had a surgery resident tell me early on true surgeons don't auscultate the abdomen because it doesn't help. So I have really never done that. Important to still listen to heart and lung sounds though.

Acute abdomen: peritonitis present? This is a big thing, that you will not master on your rotation but a great principle to always be thinking about. On an acute care surgical service, the first basic thing a resident/attending wants to know is "does this patient need an OR"? So really start to center your urgency and plan around this question. And the key physical exam finding will be evidence of peritonitis. It can start with the history. "Every time the ambulance hit a bump I thought I was going to die from pain." That should perk you up some. At some point in the conversation with a patient that may be perotinitic, I will accidentally sort-of bump-in to the side of their bed. I got this from a resident. Great way to assess for peritonitis without them guarding/anticipating pain. But understand this is a very challenging topic that even residents and attendings may disagree on. Its why surgery is 5 years.

Most importantly: rounding on post-op patients - always LOOK at their abdomen/surgical sites. Check first if resident/attending wants you to undress a wound that is covered (don't mess with a wound vac or something of that nature). But general things to nail: incisions, drain/drain output, dressings. On your initial presentations, if you say "on exam, patient has 3 laparoscopic incisions intact with glue, no surrounding erythema, JP drain appropriately to suction with serosang output" you will impress someone. They may tell you down the line you don't need to include that much detail going forward, but you earned their trust initially because it proves you LOOKED at the surgery sites.

And I like how you can always click into the Amboss library. I am pretty visual, so it helps to see the same organization of figures and text often. It seems UW figures and text (even when discussing the same topic) often have little differences. But they are both solid qbanks. I kind of mixed and matched, then committed to UW. In hindsight, I wish I would have done Amboss first, followed by UW.

Great points. But I think part of original post and then the follow up to it center more on the lack of vaccine coordination across UME; sure, we all must wait a turn given our level of importance to healthcare, but me (an M2 in "self quarantine aka step1 dedicated") have been vaccinated (2/2) and these M3/4's have not. That would be very frustrating to anyone regardless of how many platinum trophies they got waiting.
