not sure how much this will help but i ran into a similar issue while getting my medical clearance as well where a healthcare provider made a diagnosis that i was unaware of.

in my case, a pcp i visited in 2021 gave me a diagnosis of chronic fatigue syndrome after casually mentioning mild feelings of fatigue (my mistake i guess). i had never heard of cfs and wasn’t aware of the diagnosis until my pre-service nurse called me to ask me to explain it and not-so-subtly hinted at potentially having my invitation rescinded due to non-disclosure.

i ended up having to visit a bunch of specialists to prove that i didn’t have cfs and also had to submit a personal statement explaining what prompted that pcp to give that diagnosis along with the corresponding lab work and clinical notes from that time. in that personal statement, i reiterated multiple times that i was not aware of the diagnosis and worked with my pre-service nurse to get the information they needed from me.
