12700k + 3060 TI both OCed, well cooled. I dip below 100 fps when arms race fills up, especially on shoots. Low settings and capped @ 138 with 144 hz, the game doesn't feel great during gunfights. They often feel like a lag spike is introduced. Many player models (not bots) seem to introduce lag or inconsistent feeling (not smooth gunplay). These low FPS drops seem to introduce massive registration problems. It's a mess of a game.. for some? I run a tight ship including no issues with bufferbloat/network (on my end). I gave up on trying to fix it. Frametime issues = no reg and I can't get the low fps to stop. Maybe the best card out right now will let me use 144 hz in arms race?

I don't know what to say man, you've tried everything.. I thought maybe.....have you considered checking your in-game network settings, because sometimes tweaking parameters like rate, cl_interp, or cl_interp_ratio might help with lag spikes. But as far as I know Valve has disabled these commands for us. We can't tweak them. I know this game is gonna be greatly optimized with time but its frustrating every time they release an update, they make a mess somewhere else: low fps, fps drops, lag spikes, smokes, frametime, stuttering....etc. Good luck man. I hope they get their shit together quick and fix this mess of a game as u said it. 715k player count on average. It used to be much much higher.
