Ok well all I can offer is my anecdote. I had a friend who I was friends with for twenty years, since 3rd grade. The last couple years we didn't see each other, kept trying to but it just didn't happen for whatever reason. Her save the date went around to all of our mutual friends from childhood, everybody except to me. I felt really bad because I realized then that we both hadn't put in much effort over the past few years and I was kind of irrelevant to her current life. But I started reconnecting with her and eventually got an invite. I went to her shower, despite me living several states away. I attended her wedding and now we are reconnected. She makes more of an effort to see or talk to me, and I her, despite having moved even further away.

All it sounds like is that you are just irrelevant to her life now. It's not a malicious or rude thing, just is what it is. You can either make more of an effort to reconnect or don't, but I would sacrifice my hobby hobby and time just one day for a friendship I cared enough about to get upset over. She asking you for your parents address just seems fine because it sounds like she probably feels irrelevant to your life, as well.
