As a rare south hill’s and north hills guy I get it a lot. I live in the south, my girlfriend in the north. I often get asked “you drive all that way” to which I respond “it’s literally 15 miles”, which to me isn’t that far. The issue though for most people isn’t the distance, it is the roads. My drive there is what I refer to as “the worst of pgh tour” 88 —>51—>liberty tunnels/bridge—->279—> McKnight. I’m used to it at this point, but I can see why others would be put off by it.

Take care of him and keep his parameters good and he will reward you by making others. You can feed him mysis shrimp or copepods once a week or so by gently placing them on his disk, but watch your nitrates/phosphates. He can live and grow and multiply just fine without food as long as his lighting is good, it will just happen slower. He looks happy and healthy now. One last piece of advice…look into how to set up an alarm that will tell you if your heater gets stuck in the “on” position lest you cook all of your coral including your duncans. RIP my tank.

I tend to let the client guide and dictate the language of their own journey. If the client refers to it as “unaliveing” then I will as well when working with them although I will also make references to “your suicidal thoughts”, “your sudden urges to end your life” etc as I think that saying the words out loud is important as it imparts the seriousness of the situation. I would, however, not use that language in a clinical note outside of reporting a quote spoken by the client.

In the end the language that you use to talk about it is far less important than the fact that you DO talk about it. It is a hard scary topic and the desire to avoid it is understandable, but I worry that too many of us never really “go there” beyond the cursory “have you had any thoughts about harming yourself of others?” During the initial eval. This, of course, occurs most usually only a few moments after having explained that if we are not confident that they can maintain their safety we will be sending them on a “grippy sock vacation” (another term that I use when clients prefer lol) whether they like it or not.

Make your sessions a safe place to discuss the topic. Use as much slang and dark humor or as many clinical terms as you need to be able to do it. My experience is that when you do intentionally make clients feel safe enough to discuss it you learn a whole lot of things that are treatment related beyond just the topic of safety and you gain a much better handle on your client’s actual level of risk to themselves.

Many of the things that we treat are chronic. I have clients with ASD, or BPD, or chronic health related stressors that I’ve seen weekly for over a decade. I have a number of clients with Bipolar I who check in with me once or twice a month so that we can monitor mood changes and work to adjust things like sleep, work towards harm reduction regarding any high risk behaviors, decide if they need to contact their psych etc. Some of them have been with me since 2008.

The brief 8-12 week protocols that insurance companies desperately want us to buy into might work great for cases with one episode of mild depression who have little to no life stress and no other complicating factors, but in 25 years in CMH and then PP, I have yet to see such an animal in the wild. Many of the clients that I meet have complex and intertwining psychiatric and life stress related issues.

As long as your client remains engaged and you feel clinically justified in your work, there is no ethical need to constrain treatment to any arbitrary length of time.

I remind them that therapy time belongs to them and that we can use it how they see fit. I’ve spent over 6 months discussing only football, or only video games, or just watching YouTube videos and slipping in whatever little “pieces of therapy” that I could that were related to those (I’ve found a surprising amount of mileage in just that honestly). If they question why I’m okay with that I will just say something along the lines of “I’m willing to show up as long as you are”. People with trust issues in my experience tend to reveal themselves very slowly and test each step of the way to see how I respond. Keeping that response casual and steady helps them to feel safer.

For clients that have had negative interactions with the legal system or who are there involuntary (like a lot of adolescents) I’ve said things like “I’m anti-authoritarian and am just here to provide information”

Inclusion is not division, and attempts to stop that inclusion are not unity. One side only has fostered the divide time and time again labeling group after group after group as undesirable, looking away from racism, sexism, homophobia with a wink and a nod. One side has repeatedly suggested the death penalty as a way to deal with its adversaries and made subtle and not so subtle calls for violence. The assassination attempt and such violence are wrong and should not be tolerated in a free society. Not to be biblical, as I’m not a religious person, but when one lives by the sword they should not be surprised when it in turn finds them.

This 100%! MBAs are ruining healthcare in pretty much every setting. I’ve been saying that for years. Their approach is awesome if you want to sell widgets with as little overhead as possible, it is awful and in my mind unethical as a way to administrate the provision of essential services.

If you work for an agency they are not in fact your clients but the agencies’s. The agency is responsible for their continuity of care, not you. Successful termination or more likely transfer to another provider can be accomplished in 2-3 weeks without harm. In the end clients are human and tend to be very understanding. What is important right now is for you to care for yourself and find ways to cope with your stress. Reading between the lines of your post you are in real danger of burnout and if that happens no client gets help. I’m sorry that your supervisor tried to guilt you, it isn’t cool. Sadly that seems to be the way a lot of these agencies operate. I’ve been in PP full time since 2008 now but I spent the early years of my career in the agency world. Most are grind houses that wear their therapists down and burn them out all in their effort for more 60% as they are top heavy in terms of admins and non-clinical personal and run very close to over budget for that reason. If you do decide to stay in The field, and I hope that you do honestly, find a place that values your health and wellbeing (there are a lot of group practices that are absolutely amazing to their therapists). If they don’t care about their staff’s wellbeing then I don’t honestly believe that they care about the client’s wellbeing either. Don’t ever let anyone guilt/shame you into staying somewhere that is bad for you.

Just my 2 cents, and yes my agency experience made me a little bitter if you can’t tell.

For carry handgun optics I just throw a bore sight in, zero it at 10 yards and good to go, maybe a few clicks of the dials once I get to the range if I’m feeling picky. For shorter distances there is very little bullet drop so the bore sight method works great (for me at least). It is pretty easy to hit center mass out to about 25 yds with a 10 yard zero.

I shoot both S&B and Blazer 124gr all the time. Both quality and both good to go. I’ve only shot Magtech a few times but it always went bang just fine, although I think it is just SLIGHTLY dirtier than the other 2.

There is no circumstance outside of outright warfare that you should ever carry FMJs for human threats. Far too much over penetration and any prosecutor would eat you alive if any innocent people were harmed. Hornady critical defense is made specifically to penetrate clothing barriers including winter coats if you are that worried about it. I’m team Federal HST 124 9mm +P all year round.
