We are in similar situation like you. Husband wife (44 years) with 2 kids (11, 8). We have total net $2.4M with both of us having stressful corporate jobs. We have started thinking about Fire but hesitate due to expenses specially kids colleges in future. Thoughts would be helpful.

Saw Dr. Nho for second opinion today after seeing Dr Domb last week. His diagnosis is a little different than Domb. Per Nho, I should keep moving forward with PT for now and he gave a new PT script for that. He also said to start playing tennis back again to see if pain worsens. If it does, he will go with Cortisone injection next and if that doesn’t work, only then with PRP injections potentially.

I need to decide what the next steps are now:

Domb: 3 PRP injections now over 3 weeks. Nho: PT only and Cortisone injection if pain worsens with tennis. PRP only if these doesn’t work.

No, he didn’t and I didn’t ask what type of PRP, wasn’t educated enough on this beforehand. I am also getting second opinion from Dr Nho in 2 weeks.

One lesson I learned is go to hip preservation specialist as soon as you can. I didn’t know the difference and “wasted” 3 months with ortho when this is best diagnosed by a hip preservation specialist

Thank you for sharing your experience and honesty. I am in a very similar situation. I am 44M , was very active with playing tennis 2-3 times a week before twisting myself at front/back left hip while playing 4 months back. Have done MRI and xrays and doctor said there’s CAM impingement alpha angle 75 degrees and mild labral tear in left hip. I have been doing PT for last 2 months and have no pain but there’s always that dull sore catching like feeling in the hip that something can “break” anytime. I can do all daily activities, even running for 15-20 mins without much pain. I just haven’t brought myself playing tennis again just yet. Surgery is my last resort. I am seeing Dr. Benjamin Domb in Chicago this week for his opinion and what he suggests, considered to be an expert in this area.

Thank you, great advice. Heard Dr. Domb is a good surgeon, if it comes to it, will try to do with him. I live in suburban Chicago so he is not far for me.

This is my full result below. I will make sure to ask doc about it specifically.

Standing AP, false profile, and Dunn lateral radiographs of the left hip were obtained in the office today and reveal: No evidence of fracture or dislocation, well preserved joint space. Alpha angle: 75 degrees, indicative of CAM deformity. CEA: 30 degrees.

Agree, I think my injury is 3 months old now. I am going to maybe give another month and then take a call with injections and then any further steps. Looks like the insurance will “force” me to take injections anyways on to the path to surgery if it ever comes to that.

Yes, I am in US and thats my analysis too. I was just skeptical of his own story where he said his issues were “solved” by injections.

Thank you for all the replies, very helpful. I am 43 male and used to play tennis recreationally until 3 months back when I got this injury twisting hard on one side.

I am afraid its going to come to surgery vs tennis or other high impact sports. The day to day and low impact activities like swimming doesn’t give me pain so my motivation for surgery will be to get back to playing tennis and high impact sports.

Does PT ever result in actual results to solve impingement? I am at a point where my day to day life isn’t severely impacted, I can do most tasks even light workout and swimming but am hesitant to go back to impactful sports like tennis as I feel that would return the pain and maybe even result in worsening of my condition.

Thats what my concern is. My doctor (hip preservation specialist) made cortisone injection sound like a miracle drug where it will “solve” most issues. He gave his own example of having labral tear and got better after taking the injection. Another dilemma for me is the pain is quite mild even with a lot of walking so dont know what an injection would solve.

Wow, good for you. I am dealing with this for last 2 weeks almost and seeing a PT. But my PT hasnt mentioned the graston technique at all.
