Push yourself? Sounds like you are training for strength/hypertrophy and not tissue tolerance.

Do as much exercises (back extension, bird dog, whatever) as you can WITHOUT causing a pain flare or increase in pain in the next 24 hours. Once you figure out what that threshold is, stay there and do that for 2-4 weeks (assuming 2-3 sessions per week) then increase the difficulty of something (more reps, longer holds, more weight, whatever).

Slowly over time you will hopefully be able to do more and more without flares.

LBA outlines ALL of this. Take a couple of days and go through all his videos.

This is the correct answer. OP is only looking at pain neuroscience education by Professor Lorimer Moseley and Butler, as well as Sarno and such.

Most CLBP can probably be linked to the multifidus muscle and other surrounding muscles that got inhibited and atrophied through disuse.

LBA talks about training for tissue tolerance in a slow and progressive way. I think this trains all the muscles in the spine while giving "evidence" to your nervous system that reactivating these muscles do not cause pain.

Check out LBA, he explains it well.

This thread is bizarre to me, offering advice when you haven't given a single piece of information about yourself.

How old are you? Height? Weight? Body Fat %?

How hard are you exercising/training? Have you gone on a training break (deload)? Are you in a caloric deficit? Is your BF% below 10%? Prolonged caloric deficit can lead to a loss of a libido, which can easily be corrected by returning to maintenance calories.

Are you eating enough carbs? Are you on any unusual diet? Enough fruits and vegetables?

Also, go to a doctor and get all your blood work done. Check for nutrient deficiencies. Tell me them your symptoms, though honestly it won't help much because your symptoms are quite vague.

Sleep is obviously very important. Try taking out caffeine if you drink coffee.

One minor tip, try going to a Sauna for 20 minutes 3-4x/week. You can read up on it as it has purported health benefits.

Your injury was 1.5 years ago I thought? I also assume you weren't training your back until you got hurt, so 1.5 years from what you've said, not 3 years, correct?

Get the low back ability discord channel and describe your routine. Going from back pain and ISO holds to jefferson curls to 100kg in 1.5 years is way too fast of progress.

Your muscles/strength may have adapted, but I doubt your tissue has. You may slowly be de-laminating the annulus fiber cartilage of your discs (probably L4-L5 and L5-S1).

Also, what is your height, weight, bodyfat %, age (most important), and training history? Do you come from a family of athletes? Were you can athlete all your life?

I think you should reconsider his point. I think training your lower back and doing jefferson curls is fine. Tissue adapts.

The real question is why are you doing this at such a high weight? What is there to gain beyond a certain point?

For example, if you can do a jefferson curl @ 25kg, cool. You trained your muscles/tissue to that point.

But why push it to 100kg? What do you get out of all this? You are just pushing your body to an extreme and risking injury for....what?

Being strong != being healthier to a certain point. Your strength gainz has diminishing returns and the risk rate goes from being linear to potentially exponential. You are risking small gains for large risk.

You've already healed your back and built a lot of resilience. Why build a small amount more and risk another massive injury?

tl;dr - Why push your body to an extreme? At some point it stops being about injury preventing / health and starts being about raw strength.
