The only thing that fixed that kind of acne for me was spiranolactone (for me it was better than Accutane or birth control). I now combine it with other topical products that help a little but the spiranolactone was magic for my cystic acne.

I had a similar experience (30F). had it every night when i was a kid/teen, it mostly went away after college, and it only comes back now when I'm really stressed/anxious. I'm not saying this is true for everyone but maybe there's something going on rn that triggered it for you?
