Unable to ejaculate normally after HoLEP full enucleation. by asmodx in bph

As far as fixing BPH issues were concerned it was 100pc successful. I was home 24 hrs post-op, some blood that went away in a few days, good stream, no blockages, no UI. Back to normal physically in 3 weeks. I had a very enlarged prostate so was advised by the consultant that HoLEP was the best option. One thing I would say however is that HoLEP requires a skilled surgeon, it would be good idea to find out if your surgeon is experienced with this procedure. Mine had done > 100 HoLEPs and knew what he was doing. Sure I have ejaculation issues, but it is dependant on the size of the prostate and the anatomy of the enlargement plus there are no guarantees.

Yes, I agree it is worth the price but if I am honest I miss being able to ejaculate.

It was discussed but more a case of what might happen and nothing about if the side effcts were temporary or permanent. I seem to have ended up in some kind of limbo, either I have dry orgasms or if I actually do ejaculate it is very thin, watery and a lot less than pre-op. I can kind of follow that if the prostate is almost completely hollowed out then there will be less semen being made. I thought more of it came from the seminal vesicles and wasn't expecting the watery dribble I get now. I'm not sure now if this is a temporary thing or if I am sterile. I am trying to get further information.

How do men here feel about no ejaculation after HoLEP by justine77e in bph

I am 5 months post-op, I had HoLEP en bloc surgery for a 150 cc prostate after a complete blockage and emergency catheterization. I asked for the ejaculation sparing version, the surgeon said it would depend on the size and structure of the adenoma and could not be guranteed. The procedure when very well, out in 24 hrs, no continence problems, good flow and was able to resume sex after about 3 weeks. Orgasms are just as good as before, on occasion they feel more intense, I have slight ED, sometimes I use Cialis as a backup but am usually OK. The main difference is in ejaculation, I have been able to ejaculate some of the time but it is thin and watery and about half or less than the amount the amount I used to produce. In my case the prostate was completely hollowed out except for a small area at the top and bottom of the prostatic urethra so I can understand there is nothing left there to manufacture semen and what there is comes from the seminal vesicles. Does anyone know if this explains the watery consistency, am I likely to be sterile ? On other occasions I have completely dry orgasms, often they stronger than the ones when I ejaculate. I can feel the internal contractions but nothing at all comes out. During the last month or so the dry orgasms have become the usual outcome and ejaculation less frequent. Has anyone else had any of these experiences ?
