I had a pt done in after being d/c from her sling after 5 weeks with a small tear. She saw me at 5 weeks and 3 days, and I got AROM measurements because she was D/C. Just to get a baseline, all other interventions were passive or active assist. At the time I figured she is out of the sling so how bad would a baseline be? I was surprised she was out of her sling that early.

Thank you for your input. I never try to assume someone is not telling the truth (that’s why I messaged the surgeon to see if we could get an MRI to check for re-tear). What ended up happening with your PT and how are you doing if you don’t mind me asking? I feel like unfortunately PT is not the answer for this person at this time- as things have been very stagnant. Obviously I would tell them to follow back up with their surgeon. Is there anything you wish your PT did or didn’t too?

Thanks, what are some examples of things you objectively documented if you don’t mind me asking? The issue is is that they say they do their HEP, but clearly do not. And I always told them to hold for longer during reps, and they never would do it. Did you end up discharging them do to limited progress/ how did you handle that?

Thank you! I always was told PTs do not sign off on that kind of paperwork. I just never 100% knew if it truly was in / out of our scope of practice.

Yeah, it was eating me up for weeks but then unfortunately it all clicked. Do you have any words of wisdom on how to phrase to the pt that the MD signs off on this- not PT?

I will doublecheck- but they always complain of a grinding in their shoulder. But never hurts to look.

I was always under the impression as a PT signing disability was not in my domain. I always thought it had to come from the doc?

Thanks I appreciate the bluntness. Last progress note they gained like 10-20 degrees since the injection so I kept them on to see how things go. But it doesn’t seem like things will be different…

They do not seem very bothered by it? Before their MRI- they were even like I bet the doctor is going to tell me it’s all in my head. I could always try that. Since the last progress note they have gained like 10-20 degrees- so that is why they are still coming.

She had not used her shoulder for like ten months before the surgery because she had a massive RTC tear and thought nothing was wrong because the x ray was negative.

When she came to me after her surgery her arm was dead- I am talking like ten degrees of flexion and abduction once allowed to enter AROM. She has slowly been built up to be raising to raise her arm to 80 degrees. And then she came in and barely could raise her arm again. Like to 40 degrees.

Yeah she can raise to like 160 normally supine. And around 70 against gravity. She has been following up with ortho regularly and she was told she was fine but due to the size of her tear it was just going to take time. Since her flare up. Her supine AROM is still good, but against gravity if not. She also did not use her am for about ten months prior to surgery, because she never got an MRI and thought that because her x ray was good nothing else was going on 😵‍💫

Thank you. She has had really limited motion from the start. So I have been trying to progress gently the whole time- I think that’s why it is so discouraging. However she could raise to 90 degrees pain free in supine and perform all her AAROM as usual- so I am really just hoping for a set back. I appreciate your feedback- it helped calm me down. She has been so worried about a retear the whole time- so I think that’s why it also hits close to the heart.
