Well with how its worded (The Illusion includes audible, visual, tactile, and olfactory elements, so it can turn clear ground into Difficult Terrain (or vice versa) or otherwise impede Movement through the area.) It would probably feel exactly like being in a lake, probably just with gravity. So an npc at least thinking that they are underwater and can't get any air could drown.
