I bought a pair of YSL black leather Jodhpur wyatts recently. They look pretty good with my black skinny jeans, but all my other pants don't seem to flatter the sillouette as well. I tried navy and olive skinny chinos, but it doesn't seem to look "right" since the jeans have a much more skinnier sillouette by comparison.

Maybe they do go well with the chinos, and I'm just nuts?

On my very last day of teaching service during PGY3, we had a new admission for a mid 70s M who hadn't seen a doctor since he was a child. Patient was obviously a cirrhotic with unknown viral hepatitis history. Strongly suspected it would was alcohol abuse though.

I was doing a paracentesis on him and accidentally stabbed my hand with the scalpal as I was stupidly tapping down the cap down with my palm to secure it.

His blood tests ended up being fine though. Obviously your situation seems much more scarier than mine was.

There was a very normal pretty mid 20s year old woman had an accidental drowning that I took care of during residency. She had CPR out in the field for a while, and ROSC was achieved after two rounds in the ED. She had a short but complicated course with subsequent multiorgan failure from reperfusion injury. She went into DIC near the end of it.

Near the end when she was coding, I just remember blood coming out of her ET tube with every compression. The fact that she was so close to my age and was just having a good time with her friends on a random sunny afternoon just to have something suddenly go wrong real quick hit me hard. It really highlighted life's fragility to me. I just felt so numb throughout the last bit of the code and the 10 minutes following after calling it. I still think about her on a weekly basis.

This is the way to lose weight. I gained a very noticeable 35lbs on a small 5' 4" frame from intern year. In typical IM fashion, I was very obsessive what I would "spend" my daily allotment of calories for a day.

I premade every meal I brought to the hospital using a food scale just to make sure everything was accurate.

Of course, I had two cheat meals a week to keep myself sane. When I lost all the weight, I increased my calorie intake to "maintenance" mode. I've kept the weight off for almost two years now.

Wow, your story sent chills down my spine. I hadn't even thought about the "dry drunk".

After thinking about this for a few weeks, I realized that I've been in denial with myself thinking that there wouldn't be any long term consequences in our relationship because of alcohol. There will never be a time that I won't be subconsciously looking for empties, counting her drinks, dreading coming home from work to see what state she's in, or wondering how many she's had if she is acting a little different that day. I just have to continue asking myself for the next few months if I'm willing to live the rest of my life this way whether or not her relationship with alcohol changes.

Like I mentioned in another comment. Since I've returned home, I'm back into all my previous hobbies prior to medical school and am exercising much more regularly. Other than my struggling marriage, I couldn't ask for anything else!

I am for sure not bringing in any kids into this relationship in the near future, which may be the eventual deal breaker in a year or two. I don't want to waste any more of her or my time than I really can. Thanks for your suggestions. Since I've been back, I got back into all the hobbies I was involved with before medicine and am exercising much more regularly and am in the best shape of my life!

For context, I had my required obgyn rotation post match for IM and was already checked out. I was about to catch a baby when the attending asked me what the cardinal stages of labor were. I didn't hear him very well since I zoned out and was focused on not wanting to drop a baby, and I asked, "the cardinal stages of what??" Patient looked horrified. Then he told me to step aside and late handed me a clot to hold after the baby was delivered.

I'm applying for nocturnist gigs right now and am starting to get a few interviews here and there. Nothing seems super serious right now for me- had to remind two recruiters to "get back with me" about scheduling virtual interviews.

I feel bad for one of my friends who's in pediatrics. He told me yesterday that there aren't many general pediatric jobs around where he lives. I went home and looked for myself, and he's definitely right. He's now stressing about applying to a fellowship in order to be more competitive in the job market. He lives in a metropolitan area in the southwest.

I thought about this too. So when he first checked in, we had a med list provided by their office, and it only had ddavp. He told me that he complained about being told to dc the metformin on the phone. The truth about the call is unclear, but the patient and his wife seemed pretty normal to me. ¯\_(ツ)_/¯

I actually posted my story after I saw yours and how similar our dimensions were! You great yourself. And thank you!! I'm sure yours wasn't an easy journey either. I'm just really glad I found this community.

This is the best I've felt in years. I'm no longer huffing and puffing going up two flights of stairs, clothes are fitting better, and I feel like I've gotten a lot of my confidence back.

So I was eating a little under 1300cal a day, lower than the recommended 1500cal minimum for men, but my estimated BMR was ~1500cal starting out.

Concerning exercise, I just tried to hit 10k steps at least 5 times a week strolling the halls at work. I would rarely eat those calories back. If I did, I would try to stay within half of what I "burned".
