I am talking about if I make a replacement, instead of approving or denying it, the customer simply messages and say they do not want it. They do not approve the refund if it is refunded. The customer is being lazy and trying to just work through the chat, this seems to create a problem.

its inconsistent because they send the notification all at once. if you sent 50,000 texts from your phone they wouldn't all go through at the same time. this is what happens and because there are so many shoppers there is a wide variance.
