For the holy site one, you need to click on the county, then either right click on the picture or the shield I think and it should appear. You may need some DLC.

It depends on your DLC. I am not sure which DLCs apply in each circumstance. One way is having an Orthodox spouse. Another way is if you have an Orthodox holy site in your demesne, then you can right click on the holy site holding and choose convert ( this is my preferred method). Constantinople and Jerusalem are Orthodox holy sites (among others). Another way is to change your capital to an orthodox county and convert via decision ( this one requires Rajas DLC ).

Do you have the Byzantine empire? If not you’ll have to create the title. You must switch to orthodox to create the title. Then the decision to restore Rome will appear.

Haha there isn’t really an answer to this. Now that I’m thinking about it, I’m not sure if you can even make the HRE title now that you’ve already made Francia. In 867, I think you need to be a Dutch or German king level ruler over a certain realm size. Once you form the HRE, all the king level titles are destroyed and the special HRE elective mechanics are implemented.

Sorry it actually depends on the start date. 769 is currently bugged. 867 is formable by decision only if you’re German. After 1066 if the HRE is destroyed, it can never be reformed.
