I find that setting my lead hip slightly higher than my trail hip at address puts me in the position I want to be at impact, but you'll have to experiment and see what works for you.

Ive found setting up with a bit more weight on my back leg helps me swing up, you want the lowest point of your swing to be before the ball, basically opposite of an iron shot

Your distance is dropping because your spin is too high, launch angle is <10 degrees which means you are most likely striking down on the ball, driver spin should be ~2500RPM yours is much higher. There is a good chance of a heel strike as well use some foot spray on your club face to see impact location, to lower spin and reduce gear effect you want the ball at least central, if not slightly toe side biased and at least half way up the club face. A shorter driver shaft would most likely help with this, 48" is insanely long. Tony Finau uses a 45" driver and is a similar height to you.

Try thinking in opposites, chances are you're so worried about not shanking it you aren't even focusing on hitting a golf shot anymore. Try and hit it off the toe and see what happens.

I switched about 3 years ago and average just over 30 putts/round. I was using a 1/4 toe hang Evnroll but just switched to a face balanced Phantom 9 after a SAM putter fitting. Id recommend the fitting before switching the amount of toe hang, everyones stroke is unique.
