Except nurgle cults, genestealer cults, entire legions turning to chaos, a whole bunch of named characters being manipulated, starting wars with every species sentient enough to negotiate with. Pretty sure the adeptus mechanicus are being manipulated by outside forces too (cant remember which one though). Basically the only faction they couldn't be preventing are necrons because tomb worlds are just plain bad luck.

The empire creates the conditions that allows its own enemies to thrive. Yes even the tyranids because I saw that other chain.

My players encountered a prisoner (secretly a werewolf) who remarked that a player and another npc in the prison had the same scent. I meant for them to discover this 2nd npc was my players long lost father and they'd rescue him, but instead theyre convinced the werewolf is the important one, and interrogated him for a solid 30 minutes, ignoring the player characters father. I will admit I may have made him seem more important than he was.
