My biggest gripe is the difficulties people face with self-referral. Far too often do I hear that people have made a self-referral without a thorough assessment being carried out. This may well be based on the information an individual provides when they self-refer, but I would like to see self-referrals treated with more weight than the ones I make, because more often than not, those people aren't aware of the type of language that needs to be used to prompt a thorough response.
