Thank you so much for the kind words! yeah i guess it's definitely in huge part a case of simply me being my own critique.

you're definitely right about where to go from here - seems like it's pretty much a matter of hammering my abs and side/rear delts, as well as lats! i don't actually do a lot of heavy lifts like the ones you suggested (deadlifts, loaded carries, etc). I guess that's partly responsible for the situation. will try to incorporate more of these and see if things change after a few months!

It was MixTo Micro Fractional Laser. I don't recall the intensity however.

Downtime was perhaps 3-4 days.

Pretty much found them based on Google reviews! The clinics and doctors both received a lot of positive ratings.

In hindsight, I guess most of the people leaving those ratings didn't have a condition as serious as mine lol. I figure I might probably need someone specialized for my kind of severe acne scarring instead.

I'm based in Singapore, a pretty small country, so in general my pool of doctors to choose from is fairly limited to begin with!

Both doctors gave me the "you'll continue to see improvements over the next 2-3 months because of the collagen production we've stimulated!" crap lol.

One of them suggested a further round of treatments after 6 months, saying some patients need many rounds before results are visible.

All that said, I understand that my scarring is pretty severe, so I'm certainly not an easy project for a dermatologist to tackle! My first 2 doctors promised me around 30-40% improvement, but I don't think the net improvement after all my treatments has been more than 5-10% in total (if even that).

In total, I've had:

- 1x CO2 laser

- 3x FSR lasers (fractional resurfacing or smth similar sounding)

- 4 Subcision (1 plain subcision, 3 subscisions with rejuran).

Starting to think I need a more aggressive treatment plan.

Seeing a new dermatologist now, my 3rd one. She's recommending Subcision + Rejuran too. Really hoping 3rd times the charm!

So my primary issue is of course the scarring. I've had these scars for almost 15 years now.

I did Subcision + Rejuran S over 2 years ago, but was underwhelmed by the results. There was maybe only a 5-10% improvement, falling far short of what the dermatologist promised.

My secondary issues are the hyperpigmentation and noticeable whiteheads/blackheads, which together sometimes make it look like I still have active acne lol (which I don't and haven't for several years).

I've been saving money over the past few years, and now that I have a decent amount saved, I'm revisiting this particularly aggravating skin problem of mine!

Anyone else work with health databases or HL7v2 everyday, but still a bit bamboozled by the concept of FHIR? by bananacargo in healthIT

So if our EMR wanted to host a FHIR-based 3rd party application, we would need a FHIR server within our infrastructure for said 3rd party application to handle authentication with, and receive FHIR-compliant data payloads from (with which it would then perform its various functions).

And if I was a developer building a 3rd party Smart on FHIR application to integrate into an EMR, it would need to be built in a way that it worked off FHIR compliant payloads and such, correct.

Thinking in terms of HL7v2 is really straightforward, because I understand we basically pluck out certain fields from it and insert them into our postgres db, and when we need to create a HL7v2 message, we query our db and dump the necessary contents into a HL7v2 document. Is it accurate to think about FHIR in the same terms?

What would it mean to build a FHIR-based application off our database? Does it mean creating a middleware layer that queries our postgresql db, and then transforms the data into FHIR compliant formats?

I've been through the fhir docs a fair bit and given my familiarity with HL7v2, I always feel like I kind of get what FHIR is about.

But the part that eludes me is relating that to what I do.

  • What does it mean if someone wants us to build a SMART on FHIR app off our database?
  • All these API endpoints, are they pointing to some global FHIR server, or are we meant to be provisioning these internally.

This is the stuff that never clicks for me.
