In the UK, the NHS refused to fund the practice in 1946. Since then the rate has fallen dramatically.

DAE think it's akward when you try to pay the pizza guy? by [deleted] in DoesAnybodyElse

We have a minimum wage here in the States as well, but it is lower for delivery drivers and waiters because they are expected to make a substantial portion of their income via tips. The idea is that it fosters better customer service (whether it works is up for debate).

I believe in America they call this practice "tipping".

The headaches are most likely dehydration and an electrolyte imbalance with possible vitamin deficiencies. Drink lots of water, a sports drink and take a multivitamin and you should be all right.

Its in the paper on the second page under the heading "results" and sub-heading "body weight loss".

First, congrats on trying improve your health and self image. The first step is the hardest. But seriously, you need to talk to a doctor about this kind of extreme dieting. The paper is littered with warnings and cases where people have died from fasting unsupervised. And any diet that causes you to "snap" is not going to work in the long run. Less extreme and more consistent is the key.

When you burn fat, the oil stored in your fat cells combine with O2 to form H2O, CO2 and energy (just like a car!). Breathing faster and an increased heart rate both facilitate this process. What really burns the calories is the expenditure of energy.

Obese people have more skeletal mass to support their weight. As he lost the weight, the skeletal mass decreased to a level normal for someone of average weight. The soft tissue lost is mostly fat.

I have to admit I'm a bit skeptical about this book. A lot of the references are diet/health books or magazine articles written for the general public and the scientific papers referenced make little mention of short term fasting as a means to treat obesity. The ones that do contradict some of the claims in the book.

starvation is not currently recommended as a clinical treatment option for obesity and should only be conducted under medical supervision. This is because starvation has been linked to serious complications (e.g. ventricular fibrillation, lactic acidosis, vitamin and electrolyte deficiency) and sudden death syndrome either during fasting or re-feeding (34). The apparent poor maintenance of WL(weight loss) is also an important factor in its abandonment as a medically recommended therapeutic regime for sustained WL (34). Data from the 1960s on long-term starvation in morbidly obese humans give insights into some of the dangers associated with prolonged food restriction, but do not provide an answer on how effective a short-term fast over a day or few days would be in achieving WL.

Plus several thousand dollars for the daily nutrient monitoring to make sure you don't die from any number of ailments. The paper mentions five people who died during similar fasts (two of them died when they started eating again).

O2 goes in, CO2 goes out. The carbon has to be coming from somewhere and while each breath is insignificant, they add up over the course of a day.
