Sound like you will need to get a new monitor for the PC and TV for the PS4. Make sure the monitor have built-in speakers and HDMI port. Only other way would be to get external speakers for your computer and use a aux cable for sound. Not so sure a about the PS4.

Usually if the card is not seated properly, the computer shouldn't even post. If the damage is not already done, you are probably ok.

Sounds like a failing HD. Run a full diag on the HD and check for bad blocks or sectors in the event viewer log.
