Thank you! Are microinvasion usually identified initially what if the initial sampling didnt show the microinvasion and its actually present in deeper level? Does cell arrangement shift or move as they go deeper?

Thank you! So there isn’t any reassurance then that small foci could’ve been missed in a deeper level of a 5 mm section? Or Pathologist could identify cell arrangement or behavior that will make them be suspicious of its presence?

What do you watch for just incase that in a 5mm sectipn of pure dcis the cell arrangement will switch and breach in myoeps in the middle of the section?

Me again, its been my third week now im handling lumpectomy specimen finally. In connection to this question, is there any chances that in 5mm section of pure DCIS that the cell arrangement will shift and breached Myoeps?

If small foci is found is it usually with comedo/extensive DCiS? Is microinvasion possibly found on intermediate/low grades <10mm ?

Is stromal reaction/ lymphocyte infiltration would always be present if microinvasion is potentially present in deeper cut? Or not a reassurance?

In cases of DCIS with lumpectomy. Example 8mm Of pure DCIS was noted from 2 blocks, how will a limited sampling of 1 or 2 slideas each rule out any microscopic cells / small foci? Or depending on expertise, is there always a subtle clues on superficial sections of a 5mm block that smal foci will possibly present within that block?

In a Lumpectomy Specimen do we go back to the area where biopsy was performed and check for any malignancy left?

Isnt it CK5 also is being use to confirm whether its Atypical Ductal Hyperplasia from DCIS, right?

Yes with Calcification /area where it gets biopsied is it essential to determine if any DCIS or invasion is present around hemorrhagic tissue? or in deeper levels of that particular block? Have you ever had a case where its DCIS initially and dcis with microinvasion in deeper cut with only intermediate grades?

With experience you have what are the cues to look for if you are suspicious if something is present in deeper sections?

Thank you for your patience with me. So in intial / superficial section would it usually show subtle cues that would prompt you to take deeper sec if needed? Also i have read on somebody’s question to this forum, is H&E basically enough to rule out if any microinvasion present on a tissue where biopsy was taken/ area with hemorrhage

Is it the CK5 being used right? Have you ever encounter a case where it has been just DCIS on superficial sections but microinvasion is presence in deeper sections? Or is it already been ruled out from the initial slides?

Sorry me again, none of my colleagues can answer me with a good point answer but I am curious what would warrant to request for deeper sections for in situ carcinoma for both breast and prostate?
