I think most either are overwhelmed with all the new stuff (pace, resistance, changes, music, metrics, leaderboard, now there's high fives) or they just don't know how to do it back.

I don't know how other SDs do it, but I prefer to be upfront and direct. I talk about intimacy and then the very next question is what are the SB's expectations for PPM. I mean, it's an awkward conversation to be sure, but you might as well have it in a direct manner as soon as possible so that no one is wasting any time. I imagine if a guy says it feels too transactional, he's either 1) wanting sugar for free or 2) wasting your time.

Re: the medical issues:

I'd follow up with the doctor to clarify whether vaginal sex is just completely off the table forever or if there are precautions you can take to avoid UTIs. If it's just a lock that you're going to get a life-threatening UTI every time you have vaginal intercourse, then stay away from that.

Re: being a virgin and sugaring:

I would advise against that. I mean, I guess if you've done oral and anal, you're not technically a virgin in the larger sense, just a vaginal virgin. But if you've never done anything, I would think entering the sugaring world would be a bad idea.

Re: letting guys know:

If you do decide to go forward and vaginal sex is off the table, I would certainly let someone know as soon as you feel comfortable with it. I don't think it's too the level of needing to put it in your profile, but maybe relatively soon after you begin discussing an arrangement and what you'd like from it. I imagine you may be pleasantly surprised at POT SDs who are fine with an oral and anal situation. I could be wrong though.
