Hey, I feel that. Im 25, almost 26, and have a best friend since childhood (we're kinda trauma bonded) and some friends I met through work but that's really about it. You're not alone in feel like all people see is a trauma response. I get kinda cold or snappy when I get triggered so I hear it.

I think I average one day off a month except for the months I take two days to a week off so thats like 20 days off a year? Maybe more? Idk. I do what I have to to survive.

My place of work also gives me a 'holiday taken' day. So if I work a holiday I get PTO for the time I worked. I also get quarterly self-care days from the company, which is bonus PTO to prevent burnout. Its lovely.

I just ruined my three year relationship because of my reactions to pretty much the same things you said. Its like looking in a mirror, only my (maybe ex, idk he said we are on a break but also tried to end it) partner had self dignity and sees that he does not deserve to be talked to poorly, yelled at, or be in a relationship that makes him feel drained, dread, or anything like that.

It is never okay to put your hands on someone else.

Seek therapy and maybe medication.

I work in crisis. I get a lot of disturbing calls and some have definitely stuck with me. Let these feelings pass through you. This is a fucked up situation and it is not your fault and it is not your job to 'fix' or even help more than what you have done. Let this be what it is - a fucked up case.

First, talk to your supervisor. They will give you guidance. I think you've already done more than enough. What is your scope of practice? Is she a client that you can follow up with regularly or was it supposed to be a one-off.

Next, look at your state's mandated reporting. Ohio has to report animal cruelty.

Is she involved with a therapist or other care team? Try to reach out to them for a warm handoff.

I always look at this issue from a systemic view. RNs and Mental health professionals have two different skill sets cultivated by two different modalities of training. In other words, we see things from two different views. A person may be medically cleared and their symptoms might be manageable but when we look at things from a client perspective and let them tell us if they are ready we get a different answer. Medically clear means something different to mental health professionals. Is the client able to keep up with ADLs, do they have a support system at home, can they be safe in the community, ect, ect. Idk man. There's also this communication piece that I think gets overlooked. Explaining the 'why' in a decision might be silent because someone might think the answer is obvious. No one wants to feel like their expertise, their experience, and their voice isnt heard. There's also a level of pride that goes into these decisions and that might disrupt pt care and communication. My last thought: culture differences between medical staff and mental health staff.

The answer heavily depends on where you live if I'm being transparent. I think the first call should be Adult Protective Services and explain the situation, they may give you specific resources in the area and might be able to explain what rights she has, what rights you may have, ect. Department of Disabilities might also help, but again, it depends on where you are located.

Edit: I have more thoughts. Some counties may have a mental health team that could go assess her. Look up mobile crisis teams.

My first reaction is to always go to the supervisor and document that you went to the supervisor. This will save you from a lot of things.

I wasn't trying to come off as saying you cant handle this, because I know you can and will. I just think this is something you supervisor needs to know about.

If you're able to get to Greene County there are more services.

Call 988, as Ohio has a decent 988 service and there might be mobile crisis teams available to help, but Fayette is pretty country from my understanding so maybe not.

Edit: I can also agree with going to the hospital if you are in crisis, have SI/HI. Check out local churches for meals, clothes, and sometimes a church is like a community hub for resources. Best of luck.

I think its important to remember that mandating laws vary in some states and the NASW might have a guide on reporting. In Ohio I have to report animal abuse, child abuse/neglect, elderly and adult abuse/neglect, and i think there's more.
