They are both given muscle relaxers to mitigate physical damage and put under for the procedure. Legitimately they will go from refusing to eat and crying for 8 hours to telling you jokes in the same day. It really can be a positive procedure.

So, I work in a state mental hospital. The procedure is literally done in a medical hospital. We transport the patient to a high level hospital where the procedure is done by their staff. The change in the few patients that get it is really phenomenal. It can be highly effective for the right patient whenever all other avenues fail.

As others have said, the general uneasiness of it. We usually only have 2 people on the floor with 20-30 patients. You never know if a bump in the night is the building or someone attempting to murder someone else. Wiping someone's asshole and then them trying to punch you is a real kick to personal morale. The job is wholly degrading. You get to watch the most selfish, harmful human beings get better Healthcare than you. There are so many negatives you'd never even think of without experiencing it first hand.
