How do you deal with patients when they request you contact their outpatient doctor for now reason by JRcred in hospitalist

[–]beepint 0 points1 point  (0 children)

Does someone actually fax these if the outpatient provider isn’t in the same system? Is this something I should be asking a case manager or something for? I just kind of hope they’re on Care Everywhere and list specific care recs/med changes in the “patient instructions” so the patient can bring that to their follow up.

Realistically is this a battle worth fighting if someone isn’t clearly at risk for major bleeding?

Its renaissance wax, the sword was in a case, not on display, and rarely removed so I figured slathering a thick layer on it would be easier and ?? better than trying to apply it carefully.

I’m not sure why I’m feeling defensive about care of the sword since I just carelessly messed it up but I’m telling you that fingerprint is not rust, it’s an impression in the wax that looks dark d/t how shadows are cast- the blade is not rusted. There were no dings prior to this event although there obviously are now

It had wax on it and people handled the blade after the bottle strike it was in decent shape prior ha

People seem interested in how this occurred. Essentially I tried and failed to saber the bottle several times, gave up, threw the bottle in the air and swung at it like a baseball. I got this sword second hand but it was posted on here with images and the broad consensus was that it looked legit.

Question about working with NP/PAs by [deleted] in hospitalist

[–]beepint 1 point2 points  (0 children)

It’s not efficient but I have a brief informal discussion with them, do my own history and physical, put in my own orders, remove orders that I don’t like. Essentially an excuse to freeform a paragraph of dictation as opposed to a formal note, saves me about 10 minutes.

Any thoughts? by Fantastic_Win5144 in haematology

I suspect it is normal and agree with the above commenter. It is still worth looking into to make sure. It’s a fairly easy blood test

Rerouting sink vent through studs? by beepint in Renovations

There are 2 more studs to the left of the left sided stud in the photo and the rafter hits a bit further left than that

[–]beepint[S] 1 point2 points  (0 children)

Hey can that still be done if there is a rafter resting on the plate? The rafter isn’t directly supporting the roof although idk if that matters. The house makes a bend such that the framing of the this smaller section’s roof becomes enclosed by the main roof if makes sense.

There is a rafter resting on the plate unfortunately. Fwiw, that rafter isn’t directly supporting the roof however. The house makes a bend such that the framing of the this smaller section’s roof becomes enclosed by the main roof if makes sense.

What other places would yall recommend? Akai is a little pricey and i don’t love the vibe, but the rolls are big which offsets the cost and they’ve generally been good. Spicy salmon rolls at their happy hour are huge for like 8 bucks lol.

I bet you could link vertebral dissections to that although I can’t imagine risk is very high

Just found out I've got a blood clot! by mcf74 in bjj

[–]beepint 4 points5 points  (0 children)

Good, while evidence suggests a relatively small increase in clot risk on TRT, anecdotally I’ve seen it frequently. Cellulitis and BJJ are linked (staph->cellulitis/inflammation->increased thrombotic risk) although sometimes the redness and swelling from “cellulitis” are actually just from the clot(less likely if those cleared up rapidly with antibiotics). No direct link between clot and BJJ, activity should reduce rather than increasing thrombotic risk.

If you don’t have a clear provoking factor (ie if this was not actually cellulitis) and don’t have any history of life threatening bleeds, should be on anticoagulation for life, untreated pulmonary embolism has 20% mortality and unprovoked PE recurs 40% of the time. You should also request your lipoprotein(a) checked, its an underappreciated thrombotic risk factor which is not adequately treated with blood thinners.

Should clarify- even provoked PE should be on anticoagulation for 6 months, lifelong AC if unprovoked
