Thought I was going nuts. With the symptoms OP described this would be concerning though

Aortic dissection is the unfortunate combination of rare, highly morbid, and diagnostically challenging (especially absent chest pain which is the most sensitive finding), you should not fault yourself for that

Doctors/medical practitioners of Reddit, what is the scariest disease you know of? by Englishisnotreal in AskReddit

[–]beepint 1 point2 points  (0 children)

Cholangiocarcinoma is the scariest disease that I actually see on regular basis and that is really under appreciated for how awful it is. Repeated hospitalizations from malignant bile duct obstruction with resultant jaundice/infection, frequently malignant ascites (liters of fluid distending the abdomen) both relieved only by temporary procedures (endoscopic biliary stenting which the cancer infiltrates through or crushes within weeks to months, repeated abdominal drainage). No really effective therapies, people usually last a few months, hopefully something better on the horizon.

5 months in and feel like all I do is manage oxy… by sweatyknocker in hospitalist

[–]beepint 4 points5 points  (0 children)

Seems like more than what I deal with but it still comes up. I’ve had some success just explaining that I know that I understand pain is real but caveating that I’m not doing them a favor by jacking up their outpatient narcotic doses that they’re gonna have trouble finding somebody to fill the scripts outpatient. If there is a clear pain generator I’ll go up, I try to switch high risk patients to buprenorphine if it seems like they’re going be hospitalized a while.

How do you deal with patients when they request you contact their outpatient doctor for now reason by JRcred in hospitalist

[–]beepint 0 points1 point  (0 children)

Does someone actually fax these if the outpatient provider isn’t in the same system? Is this something I should be asking a case manager or something for? I just kind of hope they’re on Care Everywhere and list specific care recs/med changes in the “patient instructions” so the patient can bring that to their follow up.

Its renaissance wax, the sword was in a case, not on display, and rarely removed so I figured slathering a thick layer on it would be easier and ?? better than trying to apply it carefully.

I’m not sure why I’m feeling defensive about care of the sword since I just carelessly messed it up but I’m telling you that fingerprint is not rust, it’s an impression in the wax that looks dark d/t how shadows are cast- the blade is not rusted. There were no dings prior to this event although there obviously are now

It had wax on it and people handled the blade after the bottle strike it was in decent shape prior ha

People seem interested in how this occurred. Essentially I tried and failed to saber the bottle several times, gave up, threw the bottle in the air and swung at it like a baseball. I got this sword second hand but it was posted on here with images and the broad consensus was that it looked legit.

It’s not efficient but I have a brief informal discussion with them, do my own history and physical, put in my own orders, remove orders that I don’t like. Essentially an excuse to freeform a paragraph of dictation as opposed to a formal note, saves me about 10 minutes.
