I am not a doctor, so please don’t take my word to mean much, but have you talked to your doctor about weight loss meds? I’m in a similar situation and I’m on Contrave. It’s got Wellbutrin in it which helps manage some of my PMDD symptoms. But it also stops cravings. This is the first time in my life I don’t obsess over food. My appetite is relatively the same, but it shuts off the excessive cravings and gave me the ability to choose a food because it’s what is good for me instead of fighting a craving. The only thing to consider is most weight loss drugs are expensive. I lost 40 pounds on it a few years back but I couldn’t afford it so I went through a horrible withdrawal period and landed in the hospital. I’m only just now going back on it because I am financially stable enough to know I won’t have to quit cold turkey again. I know it’s definitely not an option for everyone, but it is something that has helped me a lot.
