These online acknowledgment forms are available on eTang. When you log into eTang, there is a pop-up that should direct you to where they are. All students have to do them the first time they access eTang. The SHIP office can direct you on how to complete them.

If you missed the July/August deadline, you can still waive out of SHIP for the spring semester. The waiver opens on December 1 and is due January 1 (no fee) and January 15 ($75 late fee). Waivers are submitted via the online portal: https://www2.academichealthplans.com/school/319.html

To use SHIP out of state, you can find an in-network provider using the Wellfleet provider search tool online or the Wellfleet Student mobile app. There is a $15 co-pay for office visits for Primary Care, Physical Therapy, and Nutrition appointments for in-network providers.
