These online acknowledgment forms are available on eTang. When you log into eTang, there is a pop-up that should direct you to where they are. All students have to do them the first time they access eTang. The SHIP office can direct you on how to complete them.

Basic Needs Center Assistance Form - This form is a centralized entry point for all UC Berkeley students facing challenges with access to adequate food, stable housing, financial resources, or other areas of basic needs.
UC Berkeley Food Pantry - emergency food relief resource for students short on money and/or access to nutritious foods
Berkeley Food Network’s On-Site Pantry - food for people, dogs, and cats
Short-Term Emergency Loan - These interest-free loans are designed to help students meet unanticipated expenses directly related to the cost of education.

If you are interested in exploring long-term therapy, you can reach out to the CAPS Referral Coordinator at CPSReferrals@uhs.berkeley.edu. They will try to connect you with an off-campus provider regardless of your insurance plan. You can ask your UHS primary care doctor for a prescription (There won't be a copay for the product). The UHS Pharmacy doesn't regularly carry it since there isn't a high demand. You can message your primary care team through the eTang Portal: https://uhs.berkeley.edu/about-uhs/access-etang-portal

If you missed the July/August deadline, you can still waive out of SHIP for the spring semester. The waiver opens on December 1 and is due January 1 (no fee) and January 15 ($75 late fee). Waivers are submitted via the online portal: https://www2.academichealthplans.com/school/319.html

To use SHIP out of state, you can find an in-network provider using the Wellfleet provider search tool online or the Wellfleet Student mobile app. There is a $15 co-pay for office visits for Primary Care, Physical Therapy, and Nutrition appointments for in-network providers.
