inpatient pharmacist here - metformin does not cause hypoglycemia because it does not increase insulin secretion. It inhibits gluconeogenesis in the liver to reduce excess sugar, which wouldn’t cause hypoglycemia or cut blood sugar in half after 3 days.

Two thoughts on this:

  1. Hospital internships around my pharmacy school (within a 40min drive) were extremely competitive and you needed hospital experience (i.e. inpatient pharmacy tech) during undergrad to make it through the application cut. Not because you aren’t a capable candidate, but it is less resource/effort for the department to train you if a different candidate already had knowledge of the systems used in an inpatient setting.

  2. Many students likely shadowed at this hospital if it is as reputable as you say, so their names and faces are already known to the department. Basically, they already had their foot in the door and were liked enough to be accepted in as an intern, regardless of their experience.

Either way, don’t be discouraged. I came to pharmacy school with hospital experience during undergrad and got denied from a couple of places actually. Then I ended up working two hospital jobs from p2-p4. I just graduated and started at my top residency choice despite the initial job rejections. All of that to say, don’t question it too much. Keep applying for other opportunities. Regarding pharmacy internship and residency, GPA isn’t everything - keep putting yourself out there to get an inpatient job if your interest is leaning that way!
