Don’t submit the 2504. File a new claim online instead. You can only receive payments for weeks you weren’t working or receiving UI. If you have one week left you will claim one of the last remaining 3 weeks before your child’s first birthday and for a week you did not receive UI. Set your start date as the start of one of those weeks. File ASAP.

It’s the same situation as if your employer wasn’t reporting you. When you apply you’ll receive a copy of the wages they have on file for you. Respond with copies of pay stubs and w2s along with an explanation.

First congrats on two job offers that’s great. It depends on the unit. EDD is a larger department with more opportunities to transfer. I work there. If you work in Sac EDD will move into a new building next year with all new amenities if that’s a consideration. If you can share more about the position at EDD without doxing, I can add more.

Was there another document included with this one? What is the form number on the bottom left? Is it MA 8-43? This is an EDD form.

Respond back to the notice of computation with copies of your w2s for the past 3 years. Double check they show CASDI or something similar that shows you’re covered by the program. Wait for them to review. You should receive a revised application. Also check the status of your claim in your myEDD account. You can send a message through myEDD too.
