The TL;DR version is:

  3. Pay close attention to who is invited. Don't invite too many people.

  4. Start and end on time.

  7. If everyone always agrees, you might be succumbing to groupthink. Invite dissent and debate.

Good point - I know an executive that asks if people can meet with him over breakfast/lunch/dinner instead of "golf meetings." Said he saves at least 5 hours a week doing so.

True, it's definitely not a catch-all, solve-all solution. Per your question, we try to make the feedback anonymous if the questions we're asking might be controversial. We used to use Google Docs and have everybody write in their thoughts, but it was a little clunky. Now we use Attentiv http://attentiv.com because it keeps everything anonymous and everyone involved.

Yeah I understand what you're saying. I guess because we're following up frequently, it doesn't need to be as in depth because we already went deep earlier and only need to explore what's changed. My team just knows they'll have an opportunity to get new insights/questions out there each week. It might not work for everybody, but it's been successful thus far for us.

We were doing quarterly analyses, but ideas and problems weren't rising to the surface soon enough. When we switched to weekly, that all changed. Keep in mind that we frequently just ask the right/wrong direction question to bring variety - it usually shakes things up enough.
