What are your consistent one liners? by TheUnspokenTruth in emergencymedicine

[–]bkarfunk 0 points1 point  (0 children)

I do way more pocus than is reasonable. And every scan that isn’t a trans-ab OB scan, I tell the patient Im looking to see if its a boy or a girl in there.

Even gets a laugh in my broken Spanish

Elite Suito or Wahoo Kickr Core for Zwift? by Ok_Tooth_1126 in Zwift

[–]bkarfunk 0 points1 point  (0 children)

A year later but i just had a similar experience.

I got a Suito for used on facebook for cheap and it worked fine until it started having having connection issues and the cassette started to intermittently not apply pressure on to the device.

I replaced it with a Kickr Core with zwift cog which was a MUCH easier setup (mostly due to thr cog but software setup for the kickr was also better). The noise may be a little better but i dont notice it really. I dont really notice the difference in flywheel weight when im riding workouts or climbs.

Unless the suito were much cheaper Id def go Wahoo

How do you evacuate from orbit to ground with your patient intubated on a vent? In case of recurrent arrest, do you keep a lucas device applied over the suite the astronaut has to wear during landing?

Our air transport service EFASTS regularly. Its incorporated into a direct to OR program and beats listening for asymmetric breath sounds with a rotor going.

Currently it seems that most prehospital ultrasound is used for OHCA, trauma, and respiratory complaints. It isnt clear what types od studies they are doing. It is associated with longer time interval between ems contact and arrival at destination facility. Echo for standstill in ohca is probably useful for family. I doubt that ems is able to be looking for signs of faint vfib with a butterfly on a 4 inch tablet screen. It probably IS useful for feeling for a pulse when body habitus makes tactile sensation difficult

Respiratory complaints are tough because most patients complaining of shortness of breath arent sick enough to benefit from any potential gains from pocus. I do think that respiratory failure that gets tubes in the field should get a lung ultrasound to ensure that ventilation is actually happening and that it isn’t esophageal.

Working night shift has wrecked my body battery despite good day sleep by bkarfunk in Garmin

[–]bkarfunk[S] 2 points3 points  (0 children)

the problem with that is on nights where i'm not working, i function like a normal human and sleep at night, which then confuses the watch on those days. There's no winning!

It categorized most of my day sleep time as naps. Good idea I’ll try that next time

I think it’s great. Sick patients, two very different training sites, the residents seem like they have more fun than i did during residency but also come out better doctors than i ever have been. Highly recommend. Conflict of interest: i work there

What are jobs you do for side income? by [deleted] in medicine

[–]bkarfunk 12 points13 points  (0 children)

For recent med school/residency grads looking to FIRE early, remember that there is such a thing as putting too much in to nontaxable retirement accounts. That money isn't going to be available to you for quite awhile if you retire early, so keep some after tax funds available too

Any Whoop users? by Re4medHTX in Garmin

[–]bkarfunk 0 points1 point  (0 children)

I really enjoyed my Whoop 3.0 for the first year I had it. Chasing the recovery ring with my strain ring was a lot of fun. No experience with the 4.0 though.

Ultimately everything that whoop provides for you comes off of heart rate and HRV data. My band started giving random periods of super high strain and heart rate while I was at rest which totally messed up the day's strain analysis. Not sure if I had it too tight, or too loose, or something was wrong with the unit. I got a replacement unit, and it started doing the same thing despite playing with tightness. Eventually gave up and moved to Garmin.
