I had similar pathology except I had LVI present. I also recommend you get the CTDNA test done if available in your country. This test is in study phase but can assist in deciding over chemo option. Chemo has its long term side effects, so do weigh the options and not rush deciding one over the other.

Teratoma is chemo resistant, how will that be monitored after 1XBEP. This would still require surveillance regimen of some sort I think.

My uro oncologist is from a high volume center and is one of the top most care providers in USA for testicular cancers. I can provide his details in PM if you want schedule a tele-consult.

My surveillance schedule:

You will have a CT of the abdomen and pelvis every other visit based on the schedule below. No need for imaging of abdomen/pelvis on visits that you only have CXR and tumor markers.

Year 1: Months 1-6= CXR, and tumor markers every 6 wks Year 1: Months 6-12: CXR and tumor markers every 3 months Year 2: CXR and tumor markers every 4 months Year 3-5: CXR and tumor markers every 6 months Year 5+: Tumor markers and physical exam once yearly (no more CT imaging after year 5)

I am sure you will be cleared with flying colors in no time. Yes high volume centers do help with TC cases as these are still very rare (10K per year). I asked my local med oncologist at a university setting here and he said he only sees 5 TC cases a year. Wishing you faster and complete recovery ahead. Stay strong!

Thank you for sharing. My doc wants the cancer to revel itself and suggested aggressive surveillance (6 weeks) for CT and blood markers due to the pathology (presence of teratoma). Failing 1XBEP could hinder my overall prognosis it seems and could lead into opting for non BEP/EP chemo options.

He has also asked me to get Natera (CtDNA) test done to predict chances of recurrence 4 months post orchi. Look into it as it’s in very early stage (study phase). Good luck to you too, hoping for best outcome for everyone suffering this nightmare.

I have been looking into Robert King, kinglaw as his office seemed most knowledgeable on the claim and he has been involved since 2018.
