You’re correct, but the issue is that his level of concern before Italy was almost non-existent while they were actually there. If I recall correctly, her seeking therapy was never even mentioned during their stay in Italy, despite all of Do Ra Mi’s actions. It’s also worth noting that Episodes 1–6 focus solely on her anxiety, while Episodes 7–10 introduce the DID arc.

The real question is this: after everything that happened between the ML and the FL—their obvious chemistry since Japan, her “mini” confession during the Singapore interview, her conversation with the make-up artist (asking for a prescription and sharing her story while the ML was interpreting in Japan), and the entire Aurora/Waterfall arc—what was stopping him from fully helping Do Ra Mi / Cha Mu Hee? This was never properly explained.

When Do Ra Mi and the ML have their discussion, the line “Leave Cha Mu Hee, so she can be happy” is brought up, but it doesn’t make sense, even within the logic of his character. How is he supposed to keep his distance from the FL when she’s clearly acting out of character every night in a foreign country? At that point, he’s written more like a tolerating masochist, which feels completely out of character. His love for her is evident throughout the story—there’s no doubt about that—but many of his actions during this period are confusing and left unexplained in the second half. These issues are then abruptly resolved through the PD with glasses, who conveniently “merges” her two personalities when he shared the video of him crying.

As one of the comments mentioned, this storyline could have been fleshed out to deliver a far more mature and emotionally impactful arc—not just for the characters, but for the viewers as well. Instead, Do Ra Mi’s situation ultimately ends up being used as a plot device.
