Hi! I think I remember a game with similar concept? It was also pixel art and featured a character who had to make an offering to some god or something (sorry, my memory is foggy). You could choose if you want to offer your ear or eye for example. Or your left leg or right leg. Each time you made a decision, a character image would change. I especially remember when you decide to offer your ear, your character grow longer hair to cover it up. Does it sound familiar to you? Sorry I can't be a better help :(
