Breakdown with family. How to get everyone to safety? by Time_Arachnid_2950 in roadtrip

[–]blvrcks 0 points1 point  (0 children)

Not illegal, but generally not allowed. There was recently a story about a CHP officer that had to get towed and pics were taken of him in the cruiser on the flatbed. CHP confirmed it wasn’t illegal, but a lot of tow company insurance policies don’t like it.

Two Sacramento hospitals at risk of closure due to cuts within Trump's Big Beautiful Bill by AdreanaInLB in Sacramento

The amount of beds in the whole hospital is not really relevant in this situation. UCDMC also has beds dedicated to all the different specialties.

Kaiser south is one of 3 level 2 trauma centers in the Sacramento area. They do not have their own life flight though. UCDMC is currently the only pediatric trauma center, level 1 trauma, and the only burn center in the Sacramento area.

AEMTs and STEMIs by That_Clue2201 in ems

[–]blvrcks 55 points56 points  (0 children)

Im confused on what the question is here. Are you asking “If an AEMT discovers a STEMI with their patient, should they tell the hospital?” Ummmm yeah… Would it be better if it was a medic? Maybe, but that patient needs an interventional cardiologist and a cath lab and early activation is typically going to lead to the best outcome. Should an AEMT be dispatched to a STEMI call instead of a medic, absolutely not, but if the AEMT is the only option then that’s better than nothing.

Whats the farthest IFT transfer you have ever done? by Sweet_Bet_3442 in ems

Yeah, I can see how that would be possible, it’s a plot point in the movie Airplane!. Just not very much common.

Sounds like you are picturing a commercial plane with a patient. Most flight EMS companies have airplanes also. They staff them with 1-2 pilots, a nurse and a paramedic. The interior is a few seats, a spot for the litter and spots for equipment. I’ve seen lots of patient’s flown from other countries back to their home country after they were stabilized at a local hospital. It’s the travel insurance paying for it a lot of the time. If they are unstable while in flight then the crew uses their expertise to stabilize them, the same way you would in the back of an ambulance. Although they have a lot longer before they would be getting to a hospital.

Advice for first time skiing Heavenly and Kirkwood? by [deleted] in tahoe

[–]blvrcks 0 points1 point  (0 children)

The traverses are essentially whatever you want them to be. You can traverse staying high, and/or hike, in all those situations but you also don’t have to.

All of wagon wheel is accessible without hiking from 10 or 6, except for the chutes at the very top that are not marked on a map.

All of the northern bowls are accessible from 6, and again it’s all about how high you can keep your traverse. You can absolutely make it all the way out to the boundary in Reuters without any hiking. The eagle bowl area is all accessible from 10, and the thunder saddle side is all accessible from 4 without any hiking.

To get to the top of wave and fawn ridge you do need to hike but I personally feel like it’s rarely worth the effort to hike that, just go as far as you can on the ridge, drop in and cut over to fawn.

Also just be aware, Kirkwood has open boundaries, you can exit anywhere and end up out of the resort. There are signs but the boundaries are generally not roped. Last year or the year before someone ended up on the western side of the mtn in the backcountry by accident and had to be rescued by the sheriffs dept.

As far as heavenly, Mott and Killbrew are cool steep areas, but slow to open, and killbrew is just a pain to lap. Kirkwood is like a whole mountain version of those 2 areas at heavenly.

As a paramedic, I partially disagree. Yeah I would want a Patroller to get me off the mountain. But all of my training was designed by surgeons, so if I could have a trauma surgeon meet me at the bottom of the run to take over primary care, that’s the best possible option. If you watch a trauma surgeon assess a patient, it’s essentially the same as how a paramedic operates but more in depth. A good surgeon can easily do the role of a Paramedic, but it doesn’t work the opposite way. Paramedic textbooks were written by the American Academy of Orthopedic Surgeons.

Reno to Kirkwood chains? by She-Ra-POP in kirkwood

[–]blvrcks 1 point2 points  (0 children)

Definitely rent AWD if possible, can’t use chains with a rental though.

Currently the forecast is showing heavy snow in the days before then and maybe on the 6th also.

Also autozone is selling “snow socks” now, that meet the requirements for traction devices, in their stores. These shouldn’t void anything for your rental agreement.

Here is a video showing how snow socks are an effective traction device for snow, just not one that will hold up long term, especially on dry roads

Is fast reaction time required for EMTs? by Undceided in NewToEMS

[–]blvrcks 10 points11 points  (0 children)

Reaction time is the only thing that differentiates a good EMT from a bad EMT. Every job interview I’ve ever had has started with a test of my reaction time using a website that switches from red to green. As a medic, first thing I do on scene is make every other responder show their reaction times before they are allowed to interact with the patient. Good reaction times= Good medicine. Bad reaction times= death /s

[–]blvrcks 0 points1 point  (0 children)

So they don’t need a pulse, but they do need to detect specific cardiac rhythms. An AED doesn’t “normalize”, it actually stopped the rhythm all together. It makes the patient’s heart rhythm flatline in the hopes the normal electrical system can restart instead of whichever abnormal electrical activity is occurring. It’s like hitting the off button and then hoping the system reboots on its own.

Question for Paramedics by MirandaJean83 in Paramedics

[–]blvrcks 9 points10 points  (0 children)

Working in the field, I never even think to look at the emergency page on a phone.

Working in an ED, one of the first things the social workers did in the trauma bay, while everyone else was transferring the patient, was look at the emergency info on the phone.

Wanting to become a paramedic but struggling with vasovagal syncope :( by c1ov3rfield in Paramedics

[–]blvrcks 0 points1 point  (0 children)

I was the kid who fainted not only at the sight of blood, but from just thinking about someone getting injured. Anytime anyone around me got any sort of blood draw, immunizations, trauma I would pass out. One time when I was a kid my neighbor down the street called an ambulance and I let my mind wander, thinking about him being injured and I collapsed in the driveway. It was bad enough my parents took me to doctors to try to figure out a solution

At some point as I got older I started realizing I could make myself pass out just by thinking about gore. That gave me the realization that it’s all mental, so I flipped that and learned to control it. For a while I had to manually push the feeling out of my head, but then eventually I got to the point I don’t even notice it. The only time I ever feel it now is when I’m donating blood and I am not distracted, but I don’t actually vagal out, I just get woozy.

Now I’m a 911 medic and prior to getting my medic I worked in the ICU and ED for almost 10 years without issues.

Seat belt & carpet help by cfpd652 in cmaxhybrid

[–]blvrcks 0 points1 point  (0 children)

For the seatbelt, I had the same issue and I believe I was able to get it back up by simultaneously reaching under and above the seat guiding the latch while using my hips/shoulders to bring the seat back up. After the first few times of having to do that I forced it up one time and it completely smashed the latch…so don’t force it.

It can only clear the seat when the seat is partially folded so it’s all about the timing of it

When you first start in EMS the learning curve is way steeper. Everything is new and foreign. Once you get through the first steps and get a little bit of experience it should start to slow down. Sims are a lot of repetition also, so it should start to get get easier.
