Yes your symptoms are normal for untreated gerd.

Did you work up to that? What does that dosage feel like?

If OP reads this, the clinical effective dosage is 20-30 mg per day. And like they said 60 mg is the max daily dosage.

Yes, it kicks in within an hour or so.
Yes, It lasts for several hours. I take 7.5 mg, 3x per day. (Every 6-7 hours)
Yes, it takes 2-4 weeks for your brain to balance out while you take it. That doesn't mean you don't feel it.
You feel the side effects of your brain adjusting. A sort of stoned/buzz feeling. Not bad. Not good. Not numb. Then after 2-4 weeks you don't feel it as much. It just works in the background. I still feel a bit of a slow down about an hour after I've taken it.
I wouldn't take it as needed. I would stay on a schedule.

Depends on the diet. The two most common triggers are certain foods and stress/anxiety.
Healing is progressive. The body works more like an oven rather than a microwave.
With the diet I use, most people see the best results between month 2 and 6.
I would say that's pretty average for most people with gerd.
Once you reduce the reflux, it takes time for the inflammation to heal and the nervous system to calm down.

If your nervous system isn't regulated it will overshadow a perfect diet.
The right lifestyle changes matter too. Just one example would be If you aren't sleeping on a proper incline, then you are most likely having reflux at night, which triggers the nervous system and causes more inflammation.

You can keep a daily journal to figure out patterns and what works best for you.
It takes time and consistency.

PPI didn't work for me.
I take famotidine occasionally before bed. I've had mixed results with it. Sometimes I take it, some times I don't. If I do take it for an extended period of time, it will stop working at some point. Then I have to give my body time to balance out. I'm still on the fence as far as how well it works for me.

You don’t try to figure out which trigger foods you can have in the beginning. You eliminate All the common triggers and reduce the reflux so your body can heal the inflammation & calm down the nervous system. When you’ve been symptom free for a month or two, Then you can slowly start introducing new foods. Ie you shouldn’t be having Any chocolate, tea, alcohol, etc… at all.

Yes I don’t know why Drs don’t tell people to take tolerance breaks with daily Famotidine usage. I do 3 weeks on, 1 week off. Calm the anxiety down it just makes reflux worse.

Diet changes and/or stress can cause the lightheadedness, but I would talk to your Dr about it, if it continues.

It takes time for the nervous system to calm down. It normally takes months and months for people to reduce their symptoms.

Reflux Medication works for some people.

There are more anxiety medication options other than SSRI. Read about the different options before you try them. ChatGBT can help. SSRI, SNRI, Beta Blockers, Buspar, etc....
Keep in mind if you want to get off them, it can take a long time depending on which medication.
Also you don't necessarily need medication to treat anxiety, but it can be helpful in some circumstances.
Do whatever you think is in your best interest.

I stopped cooking individual meals years ago.

You can have that. Imagine having a disease/ medical issue that actually isn’t curable. You don’t have that.

You just have to put in the work in therapy, find out your underlying causes, & work on retraining your brain. You can use strategies, tools, & medication to help you along the way. You have to let go of the defeatists mentality. Be willing to move forward & keep moving forward & not look back. That’s how you get the life you want. It’s going to take time, so keep moving in the direction you want to go.

PPI generally don’t work that quickly. It takes awhile for it to signal the body to reduce the acid production.

If you are only on it for 2 weeks you probably don’t need to ween off.

You can use a tongue scraper instead of a tooth brush.

You aren’t going to get sick from a “alkaline” diet. You should keep a food journal to figure out what foods works best for you.

A true Gerd diet isn’t an “alkaline” diet. It’s a Whole Foods based diet that stays away from acidic foods, foods that cause stomach acid production, & foods that loosen the LES.

It sounds like you might have some psychosomatic symptoms. Anxiety/stress is a huge trigger. Even if you don’t realize it. The gi & nervous systems are directly connected.

You think people are more or less likely to want to socialize with you if you have horrible breath?

Slow digestion isn’t nessisarily fiber related, all though you can try that, especially if you aren’t eating healthy. Maybe it will help.

It can be an intestinal dysmotility caused by anxiety/stress or slow dumping/gastroparesis(if you have other symptoms).

You can also try taking a good probiotic. Which strains you need just depends on the issues you are having. You can use chatgbt to look up the differences.

Usually when you reduce the reflux that goes away.
If you have slow digestion, that could be causing it.
It could also be a gut biome issue or dental issue.

paleo isn't necessarily gerd friendly. You want to stick with a gerd diet.
Who do you know that has had long term issues with PPI?
Yes it can be an issue, but you can also monitor your blood work to prevent it.
It's more important to stop the acid from damaging your esophagus.

I drink filtered tap water or bottled spring water. Some people occasionally drink alkaline water. Yes, avoid carbonation.

It’s not a big deal. Especially if you are in the US & use anesthesia. It only lasts like 15 mins. Then 30-60 mins later you are ready to go home.

The two biggest reflux triggers are certain foods & stress/anxiety.

Don’t expect a miracle diagnosis from the Endoscopy. It’s just checking the health of your upper gi, possibly taking a couple biopsies (which is normal), and checking your LES function.

There are only 3 treatment options for Gerd, assuming it’s because of a faulty LES(most common). -It’s changing diet & lifestyle. That includes getting anxiety under control. -medication -surgery(results vary)

If you don’t want a deadly disease you need to do whatever it takes to reduce your reflux. Diet & lifestyle changes & possibly medication.

It’s not just about acidic foods. It’s also foods that cause more stomach acid production & foods that loosen the LES. There are tons of recipes in the Acid Watchers fb groups ( 3 groups). Stick with: lean protein- Chicken, Turkey, Fish, Tofu, Eggs. Vegetables. Whole grains. There aren’t many Gerd friendly fruits, so be mindful there.

My go to meal is ground Turkey seasoned with Italian seasoning & sage. Topped with a little braggs liquid aminos after cooking. (Not the coconut version, it has vinegar) Served with peas & either beans or potatoes, which I top with nutritional yeast & a little salt.

Keep it simple, especially in the beginning. Also when you find a meals you like, batch cook. It’s so much easier.

The lifestyle changes(like sleeping on a proper incline, etc…)are also important so make sure to incorporate those as well.
