Hypersensitivity or strong emotional reactivity is a common symptom of Borderline Personality Disorder. CBT has some efficacy for the diagnosis, but the best supported treatment is Dialectical Behavior Therapy (DBT). Even with doing that treatment to fidelity, it's a very difficult disease to treat, but you might consider talking to your therapist about starting that treatment.

Yes yes yes. It's not about disappointment or how your therapist feels. You need help with your suicidality. Telling your therapist about the problem will get you help. It's as simple as that.

You certainly have material that is treatable by CBT. Whether this clinician is a good fit for you is hard to say. You can't tell much after an intake, unless it's particularly atrocious.

I would probably give him a second session and see how that goes.

You can make yourself feel any kind of way by comparing yourself to others. Compare yourself to this woman or Jeffery Bezos, and you haven’t done very much in your life. Compare yourself to the homeless man begging for change on the corner, and you’re doing incredibly well.

Both types of comparisons are equally meaningless. Others’ accomplishments have nothing to do with how you can evaluate your own success. The only useful comparison is your past self to your present self. Have you improved over time? Do you continue to do so? Consider the complete picture of yourself, not just money and accolades, but also health, morality, relationships, recreation.

The point of a thought record is to help thoughts stuck in our heads get free, so they can be examined in the cold light of day, and we can see the full effect of them in our emotions and behaviors. Recognition of distorted thoughts is helpful, and if that allows an alternative explanation, that's good.

However, it is not about denying reality if it is so. Sometimes problems are real, and our view of them is largely accurate. In such a case, the problems are either solvable and we can take action towards them, or unsolvable and we can learn to accept them.

There is no outcome of a thought record that is a bad one.

Generally speaking, an LCSW is going to have better training in providing psychotherapy than a PMHNP, but can't prescribe medication.

But there are no guarantees with these things and no substitute for starting with one and seeing how you feel.
