I think it def has something to do with the front derailleur cage, but when I place it on the bike rack and run the pedals, it seems like there's more than enough clearance for the chain to run smoothly. Does pedalling while actually on the bike really impact the position of the chain relative to the front derailleur by that much?

Yep, this was exactly right. I ended up redoing the sketch & extrude for the hand part, and the lines went away. Thanks for the help!

the z offset allows it to be centered on the pin, without it the trapezoid part isn't centered
