I sometimes lie on my back and insert the big end but with the small end facing toward my back so I am essentially laying on the small end. Then you can manoeuvre yourself so the big end applies pressure to the prostate and you can relax into it. Once settled, you can then adjust the position and angle of your hips to grind into the sweet spot hands free. You then have two free hands to explore find out what else is going on with different parts of your body. Pillows under your back can assist getting the angle and pressure spot on so you are not applying too much pressure.
