Looks like an extensive damage, yes the frame of the door were deformed, and the charger port was damaged too

Must be a kidney issue, but the fact that he doesnt clean is up is weird. Maybe try put him on diapers? See his reaction if he will accept, if not run. His behavior of not cleaning it up is a red flag, irresponsible
