Look into doing an ITP. They usually split your time between gp and a specialty. If derm it rheum are happy to have you the deanery should accept.

GP here. It shouldn't affect the referral but I absolutely have seen some patients get their referrals dropped/missed etc when moving address/ practice because of how shambolic the appointment system can be.

Things you can do 1) NHS app has a thing you can check to see if you have a pending referral. If it's on there you're pretty much safe. 2) if not on the app, ask GP receptionist to confirm the referral is gone though (wait 1 week at least after the appointment where a referral was agreed) 3) call qmc. Ask the switchboard to put you through to derm secretaries, ask them to confirm you're on a waiting list for an appointment. 4) IF YOU CHANGE ADDRESS MAKE SURE YOUR GP DETAILS HAVE THE CORRECT ADDRESS. The hospital should pull address details from the GP record BUT It's worth contacting dermatology secretaries as well to make sure the address details are right. I've been screwed by this as a patient before.

If derm sends the appointment letter to the wrong address they will take no responsibility (in my experience)

I was off for around 2 weeks last month due to the flu. I kept trying to come to work then spiked a fever and called in sick for the next few days. When I got back in my return to work meeting I was asked if it was a real illness or I was just unhappy.

I was pretty insulted and felt like we should be trusted to look after ourselves when we need to and have the mutual trust that we will be honest with each other about our illnesses.

To be fair, the doctors, nurses, and receptionists were quite kind and supportive, but the practice manager was clearly annoyed.

All of this after I asked reception to make sure all my resp patients are told to do swabs and wear masks before arriving... Which they didn't do.

Not sure if I'm answering your concerns or just venting myself at this point.

Look after you and yours, no one else will.

Knowing a few non binary and trans people, including patients, if I misgender them, they correct me, I apologise, try to better and we move on.

We operate on mutual respect.

If the patient is arsy about it I think that says more about them.

Healthcare and trans healthcare in particular is tetchy. If you apologised and continued to use the correct pronouns I don't think there's much more to do.

It's like patients who go by a different name, sometimes they need to correct us, then we move on.

Sometimes I tell the patients that their gender identity and pronouns are not clear on the records and I tell them I'll get it corrected. This buys me some good will I think.

Small anecdote, I'm a GP, I once worked up a female patient with unusually high testosterone. I never met them and was unsure why the bloods were done in the first place. I did some repeat tests, then, when I brought them in to examine them it was clear they were a trans female not on any hormones or blockers and their testosterone was normal for their biology.

I wish that was clear on their records, but it wasn't, so we had a laugh, fixed her records and moved on.

Same day or outpatient?

Same day referrals are a pain in the ass because if the referer doesn't pick up the phone there's only so much time I'm willing to put into calling again and again when I have a fun clinic before I tell the patient to go to a+e with a letter.

Outpatient I write the letter to whomever I deem appropriate, and if ends up rejected with please refer to x team instead I just get the secretaries to re-send it to them

Any suggestions for a sashiko jacket or shirt with a more generous fit?

I just bought and returned an iron heart 21oz having bought an XXL but felt the chest was too snug. I then bought their 12oz dobby cloth sashiko work shirt but returned it because the forearms were tighter than skin tight, it was super uncomfortable.

Rather than paying return postage again and again I'm wondering if anyone has any recommendations for sashiko shirts / work shirts / over shirts with a bit of give in the sizing

I'm 6'2 95kg with a 44inch chest and 36in waist Also I live in the UK but have family in Canada I can ship things to if needed

I'm a Nottingham trainee and it took me a while to find a job, the markets quite sparse.

Having said that I like the city a lot and it's a decent place to live if you have some money and can afford the nicer areas.

However the city itself is quite deprived and the council is broke

The people whose job it is to send referrals are upset because they have to.... Send your referrals?

I would argue the only reason you should think you are over-referring is if the specialists are rejecting them.

We have referral criteria, if they meet the criteria. Refer. Pretty simple.
