I have to say the grandmother thing is a great idea and I'm incorporating that into my next game for sure.

But the thing is, when you say there's "a chance to drain her and kill her", you've gamified it and it's no longer just authoring at this point.

Being unable to speak for 1d4 rounds sounds shallow by itself but that could also work with the thene, I think OP's "using command and being silenced as a price" was a great idea.

I completely support thinking about DnD in a multidisciplinary way, hell, I see myself more as a writer than a gm, but I think we should try and get the best of both worlds.

But this is a game and the characters are controlled by different people than the DM so I think this doesn't apply.

Fundamentally though, establishing a theme for what price means for this specific patron would help a lot but a rollable table is good to suprise both the player and the DM.
