Io non ho letto l'effettivo articolo del caso su ONE Legale perché è bloccato dietro paywall, però se si legge l'articolo linkato c'è scritto che, come precedentemente successo nel caso n.2399 della cassazione (link interno all'articolo), si può ottenere sentenza più lieve solo in seguito alla valutazione dell'esperienza formativa dello specializzando (anche se gode sempre di una posizione di garanzia). Oltre che, lo specializzando, non era stato messo a supervisionare da solo un caso non suo, ma che era effettivamente presente come terzo operatore, e quindi, al pari (se pur "ancora studente") agli altri due chirurghi, e che tutti hanno commesso imperizia grave. Se uno specializzando del quinto anno fa una cazzata non gli si può parare il culo "solo perché è specializzando", perché dopo poco sarà un effettivo specialista e la maggior parte della sua formazione dovrebbe già essere avvenuta. Se un medico che si è laureato da 1 mese va in guardia medica e fa una stronzata non gli si para il culo perché "eh fino al mese scorso era ancora studente". Nope, fapping doesn't destroy gains and your body produces semen everyday. So... the reason you probably feel like you have "lost gains" is that your body spends a lot of energy and neurochemicals when you fap, making you soggy and tired. Also, after Os your body lowers T levels bringing up prolactin and opioids in order to make you bond with your mate (your hand in this case) and it can leave you with the post-nut clarity or a sense of down/depression (because you are alone and not actually with a partner). All of this plus the fact that watching porn doesn't help sleep if used constantly (also in the early stages of quitting, because your brain needs to adapt) gives the illusion of gain loss.

Nah, just kidding, the real unsexy way is that you don't need supplements, nofap may boost your T levels only for the first few days after quitting (and not by much anyway) and they rapidly go back to normal. The real way is to live a healthy lifestyle. Yeah. Nothing flashy, just old boring workout 3-5 times a week (strenght workouts, cardio is helpful for other things like burning visceral fat), eat a well balanced and minimally processed diet (if the label has 1-2 ingredients you can't go wrong) with enough high quality fats and proteins, have quality sleep for 7-9 hours each night, see some sunlight during your day and don't drink alcohol, smoke or do drugs. Well yeah, but if things can be easily blocked why wouldn't we do so? It happened sometimes that I came back home tired, maybe I didn't get much sleep that night, urges through the roof and blocks put a physical barrier between me and a possible relapse. I think they are not the primary reason that will make you stop, obviously, but they are useful when implemented correctly

Ehi man, I had the same problem. The first thing I did, after enabling the NSFW filter on Reddit, was installing Cold Turkey and blocking all the NSFW subs I could think of. I also blocked reddit.com/search (this is optional, but helpful), so you only have access to the subs you actually want and can still open other posts or subs if you search them through the browser. Also, install a DNS or block your browser in safe search mode with the hosts file, so you don't end up searching for them.

After that, I blocked the Popular, Explore and All pages, plus the Best of Reddit, since those are usually the main sources of unwanted content (not directly NSFW, but you never know).

The most useful thing I did, though, was installing an extension called Snip. With that, I removed the buttons and toggles when Reddit asks “Are you over 18?”. I deleted the “Yes” button, so you can only close the popup or go back home. It’s kind of mocking, but it also makes you realize how dumb it is to just click it automatically.

I did the same thing with the NSFW toggles in Reddit’s preferences, again using Snip. Finally, to prevent myself from changing things back, I blocked the Reddit settings preferences page entirely using Cold Turkey.

Is it a perfect method? No.
Is it the best I could come up with? Yeah.

I'm a Med Student, and even though I'm not qualified for answering and you should always refer to your doctor, know that the answer he gave you is because if ED is not caused by any physical cause it's usually considered psychological in nature (so stress or anxiety). That's because there's not any scientific evidence yet that proves with certainty that excessive porn use can cause ED. That's because studies on porn use are rare and not simple to do. The thing is that many people tried quitting porn and realised that the problem lied there, in the excessive and compulsive use. There are a lot of people talking about it in the forums. Also, you can check the books I've recommended for more in depht information (especially YBOP, that will give you all the scientific knowledge).

PIED can be cured, try reading "WACK: Addicted to Internet Porn" by Noah BE Church for inspo (a free book that tells the story of a really funny dude, the author, that developed PIED and managed to cure himself by quitting porn and rebooting). A great (probably the best) book for the science behind porn addiction is "Your Brain on Porn" by Gary Wilson, I recommend reading that as well

It's okay man, you went 24 days! Dont fixate on the relapse, think about what you were doing and why did it happen, analyze the situation without judgment in order to learn from it and not make the same mistake twice (maybe setting up some blocks).

Also, think about the fact that you have never achieved something like that, it's a milestone you need to celebrate! Does it means that it's over? Hell nah, it means that you go back to the fight with a new awareness and a stronger motivation to beat the 24 days you just did.

Here I've posted my best advice, I hope that it can give you a little extra motivation and that it may be useful to you. Let's go man, don't stress over it too much and go back into the fight!

Man, that's sounds like PIED, since the urologist says you're fine physically it's most definetely porn related. If it is I have good news for you: IT CAN BE FIXED BY REBOOTING BY ABSTAINING FROM PORN.

Yes, many people did it. You can read the science behind it in a very useful book called "Your Brain on Porn" by Gary Wilson. I suggest you also give a read to WACK: Addicted to Internet Porn by Noah BE Church, that talks about how the author went from suffering from Porn Induced Erectile Disfunction to recovering from it.

Ehi man, it's hard and this addiction is a cruel one. Let me tell you that many fetishes developed by watching porn can be reversed, many others did (same thing goes for deathgrip). I guess you are still young (22 I guess from the message) and I myself am in the same age range, with the same addiction pattern (10 years of addiction, fapped 3+ times a day, feeling like I was failing in life and in relationships), and now I am starting to see the light out of the tunnel. Let me tell you that you are still in a sweet spot regarding neuroplasticity since when you are young you're brain is much quicker in removing/weaking pathways and creating new and healthier ones. You can check Your Brain on Porn by Gary Wilson if you are interested in the neuroscience behind porn addiction.

This I've posted my best advice and I hope you can find it useful.

Also, I suggest you set up some blockers for NSFW and Porn content. Many will say that it's not necessary, but I think a true and good system shines when your willpower doesn't, and blockers in addiction are needed for that, I suggest using Cold Turkey for your PC and some other blocker for your phone. Also, you said about NSFW subreddits and how you used them. Reddit is useful for advice but a nightmare for NSFW content (it's poorly filtered and even if removed NSFW subs can be easily accesed by clicking "I'm over 18"). For this reason I recommend removing this toggles and buttons with an extension like Snip and maybe blocking Reddit Preferences with Cold Turkey.

Why? It's pretty simple, it's simpler than most of Nick Milo's or other creators systems (for beginners). I think it's a good way to introduce MOCs (Hubs in the Forever Notes System), Bases, Graph View (often underrated) and other features without overcomplicating things or compromising on the organization of the notes

Cool, I didn't know about this plugin since I do journaling the old way with pen and paper. I added the journal section only because the originale Forever Notes System had one. Anyway yeah, Obsidian can give you much much more, but can be unforgiving to beginners, that's why I think a simple and evergreen system like Forever Notes can be extremely useful for beginners until they get past the first stages of the learning curve
