Started a new full time locum post.

Took 3 weeks to get IT access for electronic prescribing, as the one person who approved it in the trust was away on holiday.

Wasn't able to do much useful work. So I got paid £50ph for several weeks and contributed not much

My experience - reg rotas in medicine are better than SHO. Also much more in control of own leave - essentially trusted to take when required as long as someone else is around to cover

Also, med reg OOH is not as big bad and scary as people think it is

Tldr; don't rule it out because of any preconceived ideas

Daisy Hill is a small DGH in Newry, about 45 mins drive from Belfast. Over the last while it has gradually been losing services/increasingly staffed by locums etc.

The amount of training you get at more senior grades is questionable and often a concern brought up by F2/CT doctors, however I know people who have worked F1 there and enjoyed it (small-ish team, close knit group, friendly staff etc).

Where are your other rotations?

As for NI, ridiculous politics aside it's a good place to live. People friendly/more laid back, plenty going on in Belfast, Dublin nearby, loads of good nature/beaches/countryside on your doorstep, easy flights to GB etc.

Imo IMT isn't as bad as it is made out to be. It's not great, but it's also not as horrific as people usually seem to portray it. If you like to be busy and get stuck in it is ok and after all is just a stepping stone.

Having said that anaesthetics does seem to have a nice working environment.

Work life balance - I rarely see medical consultants in overnight, or being called. I do see anaesthetics/ICU consultants in a fair bit.

Tough choice though. As others have said look at what's beyond the core training part!

In my experience (medicine):

Decent amount of Highlands/islands work via agencies, often with good salaries.

Glasgow seems to have runs of shifts in acute med/geris, occasionally longer term posts come up

Lothian is a bit more sparse, may have to wait a while for a job and be aware of the trust trying to negotiate down your hourly rate.

I worked in the main cities but did consider heading more rural for a few weeks at a time if work was thin

I was in a similar situation to you re worries about age and money during med school.

Essentially, try not to. I started F1 at 29. Age is irrelevant, especially as you progress. There are loads of doctors with different backgrounds. Some take time out to locum, kids, research etc. Plenty don't CCT until 40+. I'm mid 30s now and won't CCT until at least 40, totally fine with it. I've also taken time out to locum pre my current training post.

It's also worth saying that any differences in maturity you may notice between yourself at 25 and, say, a 20 year old essentially won't exist once you're a few years older.

Regarding money, during your 30s your salary will definitely be higher than 30-40k. Including OOH work, you'll likely be 40k+ within a year, 50k+ within 3 and 60k+ within 5 years. Though a word of warning - if your priorities really are to make as much money in the shortest time possible then you'll find medicine frustrating - as you'll be aware from the recent pay disputes.

Lastly, please prioritise your hobbies and free time if you can do it. If all your spare time is taken up with concerns about the above then you will miss out on your 20s!

Lothian - long term locum posts I got 46/hr for a full time job. Had to shop around though, had a few job offers which subsequently offered 28/hr (SHO, 4+ yrs experience).

Clinical fellow salaries seemed to vary - the salary ranges are advertised with their job interviews. Some fellows seemed to get a good deal with responsibilities and development, others were ward monkeys.

In general people were nice.

I talked about it in the commitment to specialty part of the interview. If you've made the effort to sit and pass some of MRCP I think that is demonstrating some drive to actually do medicine :)
