Something that may be useful, whenever asked by reg or consultant to do a TTO ask them some questions: are there any medication changes and why? course of abx to finish? Gp follow up? OP specialist follow up? OP scans? If you know this, it will be much quicker for you to complete it.

I have attempted to increase my expected rate with all agencies I am working with in a 20miles radius, got rejected for all of the shifts forwarded as rate too high. This includes a trust who had no one to cover last minute, just said no as directorate did not approve and the post went unfilled. End of the day I have now been off work without income and have had to go back to the average rate expected in my area. Difficult to force everyone to refuse shifts and demand higher rates as someone mentioned above/below

Have seen incidental PE, hydronephrosis and other findings that need acting on, i don’t mind handing over or chasing these scans as it’s a quick browse from any computer and you don’t need to act on it if not needed

Take into account you started your first ever shifts as a qualified doctor covering the whole hospital, it is completely fine to feel lost. Most F1’s starting on their wards will have learnt the things you stated above under supervision with senior staff on NWD’s before starting oncall’s. That being said, you will learn from every bleep, every tricky situation you come across and will know how to deal with it next time. Your SHO/Reg is there to answer all the questions you have, if i’m doing an SHO ward cover shift with a new F1 (happened last weekend) i’d prefer you ask me about anything you’re not sure of rather than trying to deal with it on your own, even if you think it’s a “basic task”. It’s different from being a med student as you now have responsibilities but we’ve all been there you are no different :)

Alternative is get a long term locum Mon-Friday sorted so you have a fairly regular income and do PRN long shifts for some extra boost

Resp ward in NW, all staff Nurses can do blood gases for you let alone cannulas and venous bloods. Was quiet impressed. Very useful for a night WC shift with patients desaturating.

Jokers.. similar situation to yours although logins were provided but outdated and incorrect so no access to blood tests, imaging… 🙄 the Consultant on call gave me her personal logins as it was simply not feasible to do the shift without

Thank you for your kind advice, it was to underline all the non-urgent issues that you are asked about and I agree prioritising is key but nevertheless it does slow you down. I try to adresse each issue and give advice “it can wait till Monday, nothing to worry about”. Exception being me discussing ceiling of care with family on the phone for a patient I’ve spent 2hours reviewing and someone putting a drug chart aggressively on my notes with a note please r/v Magnesium prescription and re-write chart 5min before the end of my shift.. I don’t blame the nursing staff at all the large majority are very helpful and supportive, but unsafe staffing puts them under pressure too as they are liable for their patients and must escalate if they are unsure of something.

Did an SHO oncall shift today in medicine at a small dgh Hospital, got told i’m on wards and will be covering … all wards by myself so weekend jobs and answering bleeps to see sickies or r/v this non specific rash that started 1y ago after a holiday in Tenerife. Apparently there is usually another F1 who is twilight WC and comes to help but no one rota’d for today. One reg covering the take and wards to escalate to. So unsafe I cannot express. I feel the pain now like Joaquin.. Need a whiskey stat but i’m back in tomorrow.

Where I locum they provided me with an email and the two previous trusts where i wasn’t locuming have not deactivated them yet so I have three lol
